Conclusions

Clinical outcomes are optimized when the clinical team is integrated from the start. A good Integrated Model includes the entire team rendering services at the clinic, from the Custodian to the Center Director. They all have interactions with the patient. Conclusion 4 Patients are less likely to drop from their care when you have an integrated staff doing the follow up.
