  • Floating Support Service – provides support for vulnerable people at risk of homelessness.
  • Family Intervention Project – provides Intensive Support for Children and Families with

complex needs who meet the troubled families criteria.

  • Move on East - provides a range of services to offenders / ex-offenders and support to those at

risk of offending or coming out of custody

  • 4Women – Womens resource center

  • Mental Health Supported Housing – support for people with severe and enduring mental health

concern

  • Liaise with others involved in the clients care and support
  • Ensure regular visits and reviews from allocated workers
  • Offer all clients the opportunity to consult about the service they receive and the right to

challenge, complain and praise

  • Provide consent based services where clients retain the right to change their mind

How we promote the service? Where do referrals come from?

Referrals coming from a variety of locations depending on the contract Referral processes vary depending on the contract but include;

  • Self Referral
  • Agency Referral
  • Criminal Justice (CRC and NPS)
  • Social Care (Adults and Children's)
  • Mental Health

  • Service Leaflets and some online presence
  • Facebook
  • Word of mouth
  • Multi-agency meetings / case conferences

  • Continuing to maintain processional resources through changes in staffing (e.g.

restructures of statutory teams)

  • Balancing the needs and wishes of our clients against contractual targets (e.g. PbR)
  • Working with clients with increasingly complex needs (e.g. LAC, complex mental health

needs)
