Is the magnification appropriate? Yes Is the section mid-sagittal? Yes Is the sample volume small (0.5 - 1 mm)? No Is the insonation angle less than 30º? Yes Is the filter low (50-70 Hz)? Yes Is the sweep speed high (3-6 waveforms)? Yes Is the a-wave normal or abnormal? Cannot be assessed The sample volume is too big. The waveform is contaminated from an overlapping signal (brighter band) which represents blood flow from the umbilical vein. This makes it not possible to assess whether the a- wave is normal or abnormal. A way of avoiding contamination from adjacent vessels is to narrow the pulsed wave sample gate and/or to place the gate in different points within the aliasing area, which identifies the ductus venosus.

Yes Is the sweep speed high (3-6 waveforms)? Yes Is the a-wave normal or abnormal? Cannot be assessed The magnification is not sufficient. For correct assessment of ductus venosus flow the magnification of the image should be such that the fetal thorax and abdomen occupy the whole screen. The waveform is contaminated from an overlapping signal (brighter band with reversed flow) which represents blood flow from the hepatic vein. This makes it not possible to assess whether the a-wave is normal or abnormal. A way of avoiding contamination from adjacent vessels is to narrow the pulsed wave sample gate and/or to place the gate in different points within the aliasing area which identifies the ductus venosus.
