LRT Process The purpose of a Local Review Team (LRT) Staffing is to resolve case specific conflicts that cannot be resolved at other Multidisciplinary Team Staffings (MDT/IPT). • Cases should be staffed through MDTs before being referred to the LRT Team – Examples: IPT (where available), HT staffing, CBC staffing, DJJ staffing, FSPT, etc. • Resolve Service Gaps – The LRT focus is on community children to prevent them from coming into care. • Types of Cases Referred – Baker Acts/Lockouts – DJJ Involved/Lockouts – Post Adoption Dissolutions – Unmet Mental Health/Substance Abuse/Behavioral Health Needs – Relative/Non-Relative placement will dissolve without additional support

LRT Process • Youth will be set up as an Emergency LRT or included on the Monthly LRT agenda. – Youth with continuing issues may be staffed monthly or more frequently based on needs. – Person requesting the LRT will be asked to present the youth’s case for discussion. – The expectation is that follow-up actions from the call will be reported back to the team • Success is based on participation by key agencies: – DCF, CBC, DJJ, SEDNET/Education, LSF/Community Mental Health Providers, AHCA, Medicaid Managed Care, GAL, Community Transition Teams and Family/Involved Caregivers

ON TARGET CIRCUIT 7 INTEGRATED PRACTICE TEAM (IPT) The Integrated Practice Team, is a community-oriented, strength based individualized planning process aimed at helping families meet their needs through informal supports and formal child welfare service systems. The goal of the IPT members is to staff cases as a TEAM and offer knowledge and expertise to develop a comprehensive family service plan WITH the family to ensure the individual needs of the family are strengthened while safely maintaining children in their homes. The type of cases referred are active investigations or open case management where there is a potential for removal of the children. The process is to identify innovative solutions to safely maintain children in their own home. BEST PRACTICE POINTS ➢ Co-facilitating by CBCs and DCF-Shared responsibilities between agencies ➢ Travel to service centers to conduct staffing-Easier access for families and staff ➢ Integration with other community stakeholders- minimizing the number of staffings for families and staff

Family Support Planning Team (FSPT) Circuit 7 Serving Volusia, Flagler, Putnam, and St. Johns counties • Link families with traditional and non-traditional services • Stabilize children in our communities and keep them out of residential treatment. • Multidisciplinary group of professionals including case managers and local community providers who will work to provide both traditional and non- traditional linkage to services including: therapeutic mentorship, behavioral analysis services, as well as other specialized therapies catering to both the mental health and social needs of the children and families being served. • The objective of CFS staffing will be to track cases in which residential treatment is required. CFS staffing will review the progress of youth receiving residential treatment and link families with transitional services to best prepare children and families for discharge.
