People in Cardiff and the Vale of Glamorgan will be able to influence decisions about health and well-being services and co-create change in health and social care public service delivery. · People with long term conditions, disabilities and mental health needs will have increased skills to engage and be involved in planning, designing and delivering services to meet their needs. · People facing inequalities will have the confidence and clearer pathways to voice their needs and support the design of services that meet their complex needs. · Service users will have increased knowledge of commissioning and procurements strategies and be able to shape the design and delivery of services. The most vulnerable and hard to reach communities will have improved routes for communication with public sector commissioning officers. · Increased voice and influence of people, communities and third sector in health and social care services across Cardiff and Vale.

Action In Caerau and Ely – Community Ambassador Project • Create an army of volunteer Ambassadors within the community of Ely and Caerau • Help community members to access, benefit from and influence services which could help improve their physical and mental health • Volunteer Ambassadors will receive a full training package and benefit from the ‘ Timeplace ’ time credits scheme • Youth Ambassadors scheme starting in schools

  5. ‘Real Lives’ and ‘Standing Up, Speaking Out’ We are a group run by and for people with learning disabilities who want to enable people like us to become self-advocates and make decisions about the services we receive. We want to make those in power involve us when they make decisions about us and how we live. Alzheimer’s Society – Dementia and Carers Project • Working with NEXUS and the Forget-Me-Not- Choir • Provide engagement activity for people with dementia whilst also undertaking consultation work with them • Employs an outreach worker to engage with people in the community around dementia and to provide signposting information and gather research.

Cardiff And Vale Action for Mental Health – Mental Health Service Users • Working with people engaged with the Hamadryad, Pendine and Amy Evans Community Mental Health Teams • Supports people with mental health issues to make their voice heard and to provide a platform for meaningful involvement in shaping services • Provides training to the staff at the CMHTs.

Cardiff Refugee and Asylum Seekers Advocacy Forum What we will do • Hold Monthly Advocacy Forum meetings to discuss and identify solutions on issues of health and wellbeing of refuges and asylum seekers • Provide asylum seekers and refugees with the opportunity to influence policies and decisions about services provided to meet their needs • Help forum members to gain better understanding of services in Cardiff and of procurement strategies. • Offer support to deliver refugee/Asylum awareness training to service providers, frontline workers to help them tailor their service delivery to meet needs of asylum and refugee communities. • Provide free training to forum members on chairing, advocacy skills and presentation etc. Target Group Refugees and Asylum Seekers Contact Person Faruk Ogut Mobile : 07929032818 Email : farukdpia@gmail.com www. dpia .org.uk

Long Term Conditions Alliance • Lead by C3SC with support from VCVS, • Creating an alliance of organisations which support people with long term conditions • Also service users who live with long term conditions • The Alliance shares information, raises awareness of long term conditions and engages in consultation processes.

Diverse Cymru – Seldom Heard Communities • Black and Minority Ethnic people • Carers • Disabled people • Lesbian, Gay and Bisexual people • Transgender people • Older people Enabling them to influence decisions about services through: • Citizen involvement forums • Training • Events • Linking with service providers and UHB • Volunteering opportunities.

  13. Hafal – Carer Participation • Mental Health Carer Participation Officer provides support to carers of individuals recovering from mental illness aged 18+, • The project enhances the voice of carers in shaping services, representing them where necessary and also provides accredited training.
