eCQM Exclusions & Exceptions

Not like the exclusions related to Core and Menu Measures. The Core and Menu Measure exclusions allow a provider to avoid reporting on a measure. Exclusions related to a CQM allow a provider to pull information out

  • f the CQM numerator or denominator because it is inappropriate.

So for example, if there is a patient in the CQM numerator and denominator for whom the CQM would normally apply, but the action would be contraindicated for some reason, the provider would make an exception to the normal care process for that patient. Rather than appear to be practicing inappropriately or not accounting for all patients for whom the CQM applies, the provider would report that person as an exclusion for purposes of the CQM.
