• 1. No maternal or fetal indication for C/S
• 2. Frank breech or complete breech
• 3. GA  34 weeks
• 4. EFW 2,000-3,500 gm.
• 5. Flexed fetal head
• 6. Adequate maternal pelvis
• 7. Previable fetus ( < 25 wk, 700 gm.)
• 8. Documented lethal fetal anomalies
• 1. Large fetus ( > 3,500 gm)
• 2. Any degree of contracted, unfavorable pelvis
• 3. Hyperextended head (> 270C)
• 4. No labor with maternal or fetal indication for delivery : PIH,

ROM > 12 hours

• 5. Uterine dysfunction
• 6. Footling breech or incomplete breech
• 7. Preterm 25-34 weeks
• 8. IUGR
• 9. Poor obstetric history, previous perinatal death or children

suffering from birth trauma

• 10. Request for sterilization
• 11. Lack of an experienced operator
• 12. Elderly primigravida
• 13. History of infertility

Cesarean delivery ข้อควรระวัง Vaginal delivery 1) Try gentle downward traction with suprapubic pressure 2) General anesthesia (Halothane groups) 3) Durhssen incision at 2, 10, 6 o’clock ถ้ามีการคลอดติดหัว 1) ขยาย incision 2) General anesthesia (Halothane group)  Matching / Grouping เตรียมเลือด  สามารถตาม วิสัญญีแพทย์, กุมารแพทย์ได้ทัน  ไม่ต้องเจาะถุงน าคร า  ไม่เร่งคลอด (Augmentation)  การเปิดของปากมดลูก > 1 ซม./ชม. ในครรภ์แรก > 1.5 ซม./ชม. ในครรภ์หลัง  ระยะ 2nd stage < 1 ชม. ในครรภ์แรก < 1/2 ชม. ในครรภ์หลัง  อย่าให้เบ่งคลอดก่อน ปากมดลูกเปิดหมด  ตัด Episiotomy ให้มากไว้  ท าเฉพาะ Assisted breech เท่านั น  คลอดศีรษะ โดย Piper forceps หรือ Mauriceau – Smelli – Veit maneauver  พร้อมส าหรับ การผ่าท้องคลอดฉุกเฉินและพร้อมส าหรับ การฟื้นคืนชีพทารกแรกเกิด Entrapment of fetal head
