Project Overview
Implement a fully sustainable integrated approach to food security within the Baw Baw Shire LGA
Progress to Date
- Continued to facilitate and coordinate Coalition working groups
- Provide support and advice to working groups for annual
planning and reporting
- Ongoing monitoring of the Coalition’s collective impact and local
food security indicators

Project Overview
- Who: HCTC applies to ALL agencies funded by DHHS to
provide health services/programs for children & families
- What: HCTC is a quality framework to ensure early I.D.
and effective responses to vulnerable children i.e. at risk
- f abuse and neglect
- When: 12 month project led by Gipps PCPs & Inner Gipps
CYAP to support agencies to implement Framework
- Where: pilot with min. 7 agencies across 6 Gipps LGAs
Progress to Date
- Governance: Project Plan, Steering Group, Project Coordinator
- Survey: baseline data re agencies’ levels of HCTC implementation
- Forum: understanding of HCTC Framework & working together
- Collaboration: expert advice from WGHG, liaison with Bendigo
Loddon/Campaspe/Central Vic PCPs
- EOIs: invited from 30 Gipps agencies to participate in pilot project
- Initial meetings: being organised with all 9 participating agencies
- Resources: developing HCTC Implementation Resource Pack
Future Directions
Project Coordinator will:
- Assist agencies to conduct initial HCTC baseline self-assessments
- Assist agencies to develop tailored, individual HCTC Implementation
Action Plans based on results of self-assessment
- Facilitate peer-learning network to support shared learnings and
resources across organisations
- Support agencies through the implementation, monitoring and
evaluation/review process (utilising Implementation Resource Pack)
- Organise training / capacity building activities
- Undertake data collation, analysis and reporting
Opportunities for Involvement
- Once pilot project is complete, we hope to roll out resources to other
agencies required to implement HCTC across Gippsland
- Even if not participating in pilot project, can start preparing for HCTC

Project Overview
Continue to embed organisational health literacy principles within the health care sector and expand principles of improved access and understanding to the human services sector.

- Currently working with LCHS, BBSC, WGHG, LRH & Wellways
- Across Gippsland 9 organsations are implementing the Gippsland Guide
to becoming a Health Literate Organsation
- Published 2 journal articles
- Launched online learning modules- Since 20 November 2017, a total of
2,976 people have completed the online health literacy training – state- wide
- NSW Department of Health to use and adapt online learning modules
- Launched mini guide

Provide cross-sector capacity building and peer networking opportunities to improve engagement between service delivery staff working in the health and human services sectors.

- Delivered 4 peer learning topics via Complex Care Network
meetings
- Provided capacity building for AOD, Mental Health and CFIFS
- 271 professionals participated in Complex Care training
- 59 professionals completed trauma informed care and
mental health training
- 2 peer networking events with 10 organisations represented

Service Coordination
Project Overview
- Support the development of the Gippsland Digital
Health Strategy, including advocating for members’ digital health needs.

between the Orange Door, human service

service coordination for people who experience family violence.
Progress to Date
- Gippsland PHN have developed an internal Digital Health Strategy
for their work.
- GHA will be completing their strategic planning process over the
coming 6 months and will be undertaking consultations with all GHA members.
- Continuing to work with Infoxchnage to upgrade S2S to the new
digital health standards – leading to increased interoperability.
- Initial meetings have occurred with The Orange Door to discuss
how we can securely share client information.
Future Directions
- PCP will continue to stay abreast of developments in
digital health initiatives and how they may support/impact any service coordination work
- ccurring in Gippsland.
- Conversations will continue between The Orange
Door to ensure we re able to communicate with each other seamlessly and securely.
