  3. NCL Commissioning We work together where we need to respond to system-wide challenges across the 5 CCGs. This is particularly where we use the same providers and working together strengthens our position as commissioners. The Governance of these groups is provided through the NCL Collaboration Board and this provides us with the structure needed to drive change and improve quality

NCL Commissioning Commissioning Intentions • Jointly commission a single provider for the NHS 111 telephone service and GP out of hours services • Collaborate in the Urgent and Emergency Care Network • Commission a psychiatric and liaison service (to include a perinatal mental health service) • Update the ‘Procedures of Limited Clinical Effectiveness’ policy to ensure that patients are provided with the best in clinical care • Collaborate on work on coding, counting and local pricing • Collaborate on acute medicines management • With the Royal Free London Hospital, redesign five key pathways to improve those services

London Borough of Camden Joint Commissioning Intentions CAMDEN PLAN 2012-17 & PRIORITES OF THE HWBB The Camden plan sets out five strategic objectives that the Council is focused on in order to deliver our vision to make Camden a better borough – a place where everyone has a chance to succeed and where nobody gets left behind.

The five objectives are summarised below: • Providing democratic and strategic leadership fit for changing times • Developing new solutions with partners to reduce inequality • Creating conditions for and harnessing the benefits of economic growth • Investing in our communities to ensure sustainable neighbourhoods • Delivering value for money services that are ‘right first time’

  7. Children’s • Integrated Care for Children with Special Educational Needs and Disabilities • All Together Better Integrated Child and Health Project • First 1,001 days • Minding the Gap for 16- 25’s • Improve the Mental Health & Well Being of Children & Adolescents

Mental Health • Improving perinatal mental health services • Implementing sufficient 24hour mental health liaison services • Consolidate funding arrangements around crisis • Female psychiatric ICU beds – • Continue the development of EIP service • Recovery college • Neuro developmental disorders pathway • Integrated Practice Unit for people living with psychosis • Develop a whole family approach in line with the resilient families partnership programme

Mental Health Cont…. • Develop business case for increased mental health nursing provision • Community drug treatment re-commissioning (Council Funded) • Reduce reliance on secondary care mental health care where patients can appropriately be in primary care. • Commissioners will collaborate with NCL strategy Carnell Farrar work to understand demand and capacity across the piece • NHS accommodation pathway placement support • Sleeping Tablets and Minor Tranquiliser Service

Reviews and deep dives in Camden specifically include NCL level review of resourcing and delivery of services Camden and Islington system review Camden Crisis Care feasibility study Redesign the future shape of: – Employment support for people with mental ill health – The community development service – IAPT provision in Camden primary care mental health landscape – Psychological therapies (IAPT) for people with long term conditions and medically unexplained symptoms alongside the PTC & Cancer Programme

Learning Difficulties • Winterbourne View Placements – people who have been in mental health hospitals (all specialist LD assessment & Treatment units) for a very long time – years. – The original target for discharge set by NHSE was April 2015. For various reasons we did not meet this but some of the barriers to discharge have now been overcome in the majority of cases and we are on target for the new target date of September 15. • Review the provision of learning disabilities beds in Dunkley ward to ensure they are achieving desired outcomes

Community • Prevention work programme for adults with long-term conditions • Self-management long-term conditions and health • Implementation of Autism Strategy • Implementation of integrated Vision / Eye-health Strategy • Co-production of Centre for Independent Living • Review of short-stay bed provision Forest Healthcare for Bridgeside Lodge step-down • Developing a single health and care reablement service • Review and benchmarking of wheelchair provision

The emphasis of the NHS will have shifted to focus on ▪ Developing a systematic approach to preventing disease ▪ Diagnosing earlier to reduce complications ▪ Reducing inequalities in health outcomes targeting vulnerable groups ▪ Encouraging individuals to take greater responsibilities for their health ▪ Supporting the self-management of illness All patients will experience ▪ Compassionate, high quality, effective and efficient care pathways that they will have been involved in shaping and evaluating ▪ Care that is integrated within and between organisations and focussed around those outcomes defined and shaped by them ▪ Easy access to services delivered in ways and settings most convenient to them For integration of care to be driven and enabled by ▪ Development and deployment of shared digital record both for clinical record sharing, data sharing, and systems measurement and evaluation ▪ Services to be commissioned and contracted in ways that support partnership and integrated working

Adults • Adult social care integrated care pilot • Community services contract renegotiation • Review and benchmarking of wheelchair provision • Telehealth development according to Camden Assistive Technology Strategy 2014-19 • Implementation of Last Years of Life Care Plan- Raising public awareness End of life decisions, Advanced Care Planning

Adults • Care Navigators Pilot Evaluation • Care Navigators to support patients requiring referral to any non-clinical services • Multi-provider integrated practice unit • Social workers in GP practice pilot • Social workers in multi-disciplinary teams (part of CICS) • Value-based commissioning IPU for diabetes

Adults • Musculoskeletal services redesign • Anticoagulation locally commisisoned serivce (atrial fibrillation) • Anti-coagulation virtual clinic • Care planning and development of a consistent case management approach across Camden • Community hypertension intermediary service • COPD service integration • GP education and population awareness to enable early diagnosis of cancer • Heart Failure integration • Public awareness of long-term conditions (hypertension)

Older People • Dementia management (memory service) • Risk stratification to identify patients to be included in the frailty register • Web-based frailty education for GPs and Patients • Complex Care locally commissioned service to identify frail patients and improve care coordination • Complex care locally commissioned service, rapid response GP service pilot evaluation • Complex care locally commissioned service over 75s home visiting pilot • Pre-frailty support to avoid complication • Community geriatricians visiting care homes and training clinicians as appropriate • Multi-disciplinary team focused on frail patients

Primary Care • Increasing GP capacity in Primary Care – improving access 8-8 • Developing the nursing workforce in Primary Care • Improving the physical health outcomes for patients with severe mental illness • Reviewing Locally Commissioned services to ensure universal coverage • Investing in IT to enable service improvement in Primary Care • Commissioning primary care services via the GP Federation to ensure borough-wide sustainability of primary care provision • Actively participating in decision-making of the North Central London Joint Co-Commissioning Board
