Applying to BTHCC Sixth Form

• Download an application form from the Sixth Form website
• Complete, save and print
• Give to your Form Tutor to complete your reference and estimated GCSE

grades

• Form Tutors will give your application form to Miss Connaughton (external

candidates to your Head of House/Head of Year/Form Tutor)

• Any queries speak to your Form Tutor / Mr Goldrick, Mr Fishwick, Mrs

• Download an Application Form from the Sixth Form

website

• Complete, save and print

Timeline for applying to BTH Sixth Form

• By Monday 30th October (1st day after half term)
• Give to your Form Tutor to complete your estimated

grades and reference

• Form Tutors only have 2 weeks to complete all

references so if you have yours completed early please hand it to them ASAP

• Ensure you complete any

application form accurately and neatly

• Write in black ink and in

capitals/type and save.

• Do not use Tippex
• Ask an adult to check your

application before submitting it – first impressions count!!!
