  2.  Contractors working on AHS property will follow AHS policies and will only use authorized entry points, elevators, bathrooms, and eating areas.

  3.  Before entering Adena Health System property, please make sure you are wearing your:  AHS Badge  Hard Hat  Safety glasses and/or other PPE  Complete the AHS Construction Daily Worksite Inspection Checklist. See attached copy. 8/14/2013 3

  4.  Provide the AHS project manager the contractor’s name and work location (inside, outside, etc.) to get a AHS badge from security. Longer term projects may get a permanent badge from the Human Resources department upon filling out the proper paperwork.  Return badges at the project’s end or pay a $25 fee. Wear AHS badges on the upper chest, facing forward and clearly visible. 8/14/2013 4

  5.  Clean and wipe off clothes and shoes before entering AHS buildings.  Do not wear clothing with rips, tears, holes, or questionable / offensive wording and/or pictures. 8/14/2013 5

  6. No smoking in any buildings or outdoor areas owned and/or operated by Adena Health System. No eating or drinking inside the construction area at any time (except designated break areas). Water is ok. 8/14/2013 6

  7.  DO NOT look into patient or procedure rooms.  Avoid foul language and/or extremely loud conversations.  Respect all AHS patients’, staff, and visitors’ privacy 8/14/2013 7

  8.  Follow the “3 second look” rule.  If there is a complaint you are looking at AHS staff, patients, and/or visitors inappropriately and/or are not following the AHS privacy guidelines, YOU WILL BE REMOVED FROM THE PROJECT 8/14/2013 8

  9.  Patients, staff and visitors ALWAYS have the right- of-way in elevators and hallways.  Clear pathways for patients, staff, and visitors.  Do not slow down day-to-day AHS operations. 8/14/2013 9

  10. Use provided hand-cleaning products often, especially if the Work space is in or near patient care. Reduce noise when possible, Especially with noisy equipment. Provide the highest level of Customer Service to our patients, staff, and visitors. 8/14/2013 10

  11. Parking areas are determined • in pre-construction meetings. Only limited company vehicles • are allowed. No personal vehicles are • allowed. 8/14/2013 11

  12.  Dispose of all waste.  Contractors requiring waste collection on site, must coordinate with their AHS Project Manager the location, size, duration and frequency of dumps. 8/14/2013 12

  13. Notify Facility Operations (779-7507) of shutdowns of electrical, plumbing, smoke detection and fire sprinklers/systems as follows.  Scheduled shutdowns require a 2-week week notifi tificatio ation in writin ting (inc nclu ludin ding g email il). ).  Unscheduled shutdowns require a 48-hour notification.  For r utili lity ty outa tages ges requir irin ing immedia mediate e noti tific icat atio ion o Contact 779-7416 06:30 – 15:30 o After hours & weekends, contact and have the Plant Operator on duty paged. 8/14/2013 13

  15.  Review the Infection Control Risk Assessment (ICRA) with the AHS project manager before the project’s start to minimize patient risk to dust, moisture and debris. The ICRA will include the start and planned completion dates, project description, and any additional requirements. 8/14/2013 15

  16.  Infection Control & Prevention (779-7568) and the AHS Project Manager and/or designee will check the interior and/or exterior project activities, and areas surrounding the project site(s), to complete an ICRA form.  The form will be signed by an Infection Control & Prevention staff member and the AHS Project Manager and/or designee. 8/14/2013 16

  17.  Infection Control & Prevention keeps a copy of the completed ICRA.  Post the original ICRA until the project is completed.  If the project exceeds the stated completion date, the AHS Project Manager and/or designee must contact Infection Control for an extension. 8/14/2013 17

  18.  Infection Control and Prevention staff, Safety staff, or the Department Director/Supervisor have the authority to stop the project if any part of the ICRA is not being done correctly.  Contact the AHS Project Manager and/or designee immediately for any problems  Restart the project when the issues are corrected. 8/14/2013 18

  19.  AHS has developed the use of Interim Life Safety Measures(ILSM) to reduce the negative impact of, or failure of, the overall Life Safety System during the course of construction renovation, or alteration activities.  Use the ILSM during project development and monitor continuously until the project’s completion. 8/14/2013 19

  20.  Obtain the ILSM from Environment of Care(779- 7416) if construction might cause a failure in existing Life Safety Code building features including, but not limited to: ◦ emergency exits ◦ Emergency access for emergency services ◦ Fire extinguisher equipment ◦ Fire alarms ◦ Fire suppression systems  Contact the Safety Officer(779-7416) for guidance if you feel the dust you create may affect the smoke alarm system Confidential Not for Distribution August 14, 2013 20

  21. The AHS Project Manager will: Work with the department director to • make sure all patients are moved before the beginning of the project. Work with AHS Plant Engineering • (779-7504) to coordinate –  HVAC system isolation (the blocking of return air grills and duct work when necessary to avoid dust contamination of surrounding areas)  Any necessary electrical outages 8/14/2013 21

  22. Portable HEPA filtration units for • negative air pressure to exhaust air. Clean and seal ceiling vents. Wipe down tools and equipment with a damp cloth or bag them before bringing them through AHS property. This includes tool/trash carts, covers, and wheels. 8/14/2013 22

  23. Completely contain the project site by  Using fire-rated plastic for all short term jobs  Taping barrier seams to make sure they are fully sealed – No drooping!  Installing temporary floor-to-deck barriers, as needed. 8/14/2013 23

  24.  Installing solid-wall barriers for longer projects.  Using tightly sealed barriers/control cubes to contain all work, including holes in the walls, ceilings and/or floors.  Properly sealing doors into barrier areas with a zipper.  Not using control cubes with broken zippers. 8/14/2013 24

  25.  Sealing wall/ceiling/floor openings with an approved UL sleeve and RED fire caulk  Doing any assembly or cutting of pipe, wood or other materials inside de the designated work area 8/14/2013 25

  26. - Using walk- off mats (sticky or moistened carpet mats) between your work area and the rest of the building - Peeling sticky mats to the next layer as soon as they are no longer sticky - Vacuuming carpet mats daily 8/14/2013 26

  27. • Using dustless drills, sanders, etc • If one is not available, collect dust as you drill to keep the dust from gathering on the floor. • Using personal protective equipment, as needed. 8/14/2013 27

  28.   29. These are the minimum PPE requirements. Additional PPE may be required based on the task and/or scope of Hard Hat(worn work performed.   30.  Removing disposable coveralls and disposable booties before leaving the demolition area.  Using HEPA vacuums (inside anteroom) to vacuum off clothing/boots upon exist of project site when disposable coveralls are not required. Workers are not allowed outside the construction area with dusty work clothes/boots. 8/14/2013 30

  31.  Establish traffic patterns to prevent dust from being taken outside the construction area and clean up all dust immediately.  Clean all areas adjacent to the project site often, using a damp mop (change water often) or HEPA vacuum, to control dust. Do not use a broom because it stirs up dust. 8/14/2013 31

  32.  Take construction site trash out daily in a covered, dust free container.  If a construction chute is used, it must be sealed to prevent the accidental spread of debris. 8/14/2013 32

  33.  Carefully remove barriers once work is near completion or completed.  Before taking barriers down, clean the work area.  Environmental Services (779-7968) should clean the completed project area carefully before allowing patients to return. 8/14/2013 33
