AHS Badge Hard Hat Safety glasses and/or other PPE Complete the AHS Construction Daily
Worksite Inspection Checklist. See attached copy.

Provide the AHS project manager the contractor’s name and
work location (inside, outside, etc.) to get a AHS badge from
- security. Longer term projects may get a permanent badge
from the Human Resources department upon filling out the proper paperwork.
Return badges at the project’s end or pay a $25 fee.
Wear AHS badges on the upper chest, facing forward and clearly visible.

Review the Infection
Control Risk Assessment (ICRA) with the AHS project manager before the project’s start to minimize patient risk to dust, moisture and debris. The ICRA will include the start and planned completion dates, project description, and any additional requirements.

Prevention (779-7568) and the AHS Project Manager and/or designee will check the interior and/or exterior project activities, and areas surrounding the project site(s), to complete an ICRA form.
The form will be signed by an Infection Control & Prevention
staff member and the AHS Project Manager and/or designee.

- f the completed ICRA.
Post the original ICRA until the project is
completed.
If the project exceeds the stated completion
date, the AHS Project Manager and/or designee must contact Infection Control for an extension.

AHS has developed the use of
Interim Life Safety Measures(ILSM) to reduce the negative impact of, or failure of, the overall Life Safety System during the course of construction renovation, or alteration activities.
Use the ILSM during project development and
monitor continuously until the project’s completion.
Obtain the ILSM from Environment of Care(779-
7416) if construction might cause a failure in existing Life Safety Code building features including, but not limited to:
- emergency exits
- Emergency access for emergency
services
- Fire extinguisher equipment
- Fire alarms
- Fire suppression systems
Contact the Safety Officer(779-7416) for guidance if you feel the
dust you create may affect the smoke alarm system

The AHS Project Manager will:

make sure all patients are moved before the beginning of the project.
- Work with AHS Plant Engineering
(779-7504) to coordinate –
HVAC system isolation (the
blocking of return air grills and duct work when necessary to avoid dust contamination of surrounding areas)

- Portable HEPA filtration units for
negative air pressure to exhaust air. Clean and seal ceiling vents. Wipe down tools and equipment with a damp cloth or bag them before bringing them through AHS property. This includes tool/trash carts, covers, and wheels.

Hard Hat(worn with bill forward unless you are welding) Safety Glasses(mus t be ANSI Z87 approved with fixed side shields Hearing protection in areas where the noise level exceeds 85db.(If you have to raise your voice to be heard by someone within 3’ of you, the noise level is above 85db. Leather hard sole work boots(Steel toed safety shoes are preferred, but not required.) Gloves(applicable for the type of work being done)
Removing disposable coveralls and
disposable booties before leaving the demolition area.
Using HEPA vacuums (inside
anteroom) to vacuum off clothing/boots upon exist of project site when disposable coveralls are not required. Workers are not allowed outside the construction area with dusty work clothes/boots.

Establish traffic patterns to prevent dust from being taken
- utside the construction area and clean up all dust
immediately.
Clean all areas adjacent to the project site often, using a
damp mop (change water often) or HEPA vacuum, to control dust. Do not use a broom because it stirs up dust.

covered, dust free container.
If a construction chute is used, it must be
sealed to prevent the accidental spread of debris.

Do not use any extremely
hazardous chemicals, aerosol sprays, or cleaning products that produce heavy fumes or odors, without first notifying the Environment of Care department(779-7416) and providing a point of contact.
Observe all safety and/or
environmental precautions when using hazardous chemicals on AHS property.
Dispose of all chemicals off site.
If you bring it – take it away with you(including empty cans, bottles, etc.)
Obtain a Hot Work
Permit for al welding, cutting, grinding or any
- ther type of work that
might create a spark.
Obtain Hot Work
Permits though Security(779-7505). Office is located within the Emergency Department area.

Once the Hot Work Permit is
- btained, the contractor must
contact Security(779-7505) in person before and after the work is completed, to sign a form indicating where the Hot Work is being performed so that the fire alarm system can be monitored
- r isolated.
The Hot Work permit must be
returned to the issuer once the Hot Work is completed.
The contractor must provide fire
extinguishers and blankets(if necessary).
Check with your Adena
Project Manager or Safety dealing with lockout/tag-
- ut of equipment before
work on the equipment is performed and a point of contact is established.
When AHS personnel
lockout/tag-out a piece of equipment for a contractor, the contractor is also required to add a lock or tag using a multilock device.
All locks must have a tag
with contact information.
− Coordinate a permit for all entries into any confined
space through your Adena Project Manager.
− The contractor will inform their Adena Project
Manager of contracted personnel procedure(s) for entering confined spaces.
− The Adena Project Manager will inform the
contractor of all the procedures and precautions in
- r near the confined space and will debrief the
contractor at the end of the job to learn of any hazards found or created during entry.
The confined space permit must include:
The space to be entered,
purpose of entry, date, and authorized duration of entry
Names of all entrants Names of all attendants The name of entry supervisor The hazards of the space to
be entered
The measures used to isolate
the space and eliminate the hazards
The acceptable entry
conditions
- The results of the initial
and periodic tests performed, with tester initials
- The rescue and emergency
services procedure and how it can be called into action
- The communication procedures
used by the authorized entrants and attendants to maintain contact during entry
- The provided testing,
communications, alarm, rescue and personal protective equipment.

- Bloodborne pathogens are
viruses (such as HIV and Hepatitis B & C) found in blood
- r certain body fluids.
- A person has to come in direct
contact, usually through a needle stick, to become infected.
- You cannot get a bloodborne
pathogen infection by casual contact.
If you are exposed to blood or body fluids:
- 1. STOP
OP what you are doing.
- 2. Dispose of any sharps
involved in an approved sharps container.
- 3. Wash the exposed area with
soap and water.
- 4. Report it to your supervisor.
