Core Characteristics of PCNs • Practices serving a population between 30 000 – 50 000 patients • Geographically to facilitate joint working with partner organisations • Organisational structure with a named Clinical Director (funded) • Sign a network collaboration agreement/contract • Opportunity to secure reimbursement for additional professional roles – pharmacists/physicians associates/social prescribers • Extended hours • Receive and manage funding for enhanced access (GP+ services)
