Accountability ( b) By January 1 of each year, the commissioner shall notify community health boards of the performance- related accountability requirements of the local public health grant for that calendar year. Performance- related accountability requirements will be comprised of a subset of the annual performance measures and will be selected in consultation with the State Community Health Services Advisory Committee. 9 9

  12. 6/30/2016 5.2.4 Monitor and Revise CHIP From Annual Reporting Instructions: Full text of measure: Monitor the strategies in the community health  improvement plan and revise as needed, in collaboration and with broad participation from stakeholders and partners. Health improvement plans are dynamic. Community health boards must  monitor the community health improvement plan, identify tasks and timelines for progress, develop changes in collaboration with partners and stakeholders, and report on progress made in implementing strategies from the plan. Your community health board reported on this measure in previous  reporting cycles. 12

  13. 6/30/2016 Reasons for selecting 5.2.4: MDH and PISC selected 5.2.4 for two consecutive years because:  Timely – Recent completion/submission of CHIP  Aligns with need to increase capacity for performance management  Emphasizes that monitoring and revising the CHIP is ongoing 13
