  22. CFD Social Worker Pilot Project • Started January 2018 • Partially Grant Funded • Paired a LSW with CFD Paramedic • Enhanced our previous referral process • Additional Focus on Opioid Epidemic

  23. CFD REFERRAL PROCESS Multiple ways to initiate a referral 1. Paramedic generates referral form after EMS Call – ES214 form 2. Paramedic submits referral through EMR 3. 72 hour call report – surveillance report

  24. CFD REFERRAL PROCESS Working Cases 1. ES 214 reviewed by paramedic & LSW - TRIAGED 2. Patient is cross referenced against existing databases 3. If found to already be linked – Case manager is contacted 4. If no connection with low acuity, case is referred to CPH

  28. OUR SUCCESSS 2016- 160 Cases referred to CPH 2017- 200 Cases referred to CPH 2018 YTD – 420 cases opened • 109 referred to CPH • 332 Cases closed YTD • 287 Cases re-linked with service provider = 68%
