The Post Falls Response Team collects consistent data at each patient fall in order to make sure that falls prevention at St. Margaret is meeting individualized patient needs.

Implementation • Resource RNs on 4B, 5A, 5B, 6A, 6B will be involved on the Falls Team • Two units every month will be assigned • Post fall the staff nurse will call the operator • The operator will page the Resource RN’s phone for the units assigned that month • The Resource RN’s on the units assigned that month will respond within 15 minutes to the fall

Duties The Post Falls Team will assist the staff nurse with:  Filling out the Fall Debriefing Form  Ensuring accurate and thorough documentation of fall event  Ensuring that Risk Master is completed  Ensuring that the MD is notified and patient assessed  Ensuring that all possible fall prevention protocols are in place
