All falls are not the same. The Post Fall Response Team provides support and suggestions. How does that help?

The Post Falls Response Team collects consistent data at each patient fall in order to make sure that falls prevention at St. Margaret is meeting individualized patient needs.

Implementation

• Resource RNs on 4B, 5A, 5B, 6A, 6B will be

involved on the Falls Team

• Two units every month will be assigned
• Post fall the staff nurse will call the operator
• The operator will page the Resource RN’s

phone for the units assigned that month

• The Resource RN’s on the units assigned that

month will respond within 15 minutes to the fall

Duties

The Post Falls Team will assist the staff nurse with:

• Filling out the Fall Debriefing Form
• Ensuring accurate and thorough documentation
• f fall event
• Ensuring that Risk Master is completed
• Ensuring that the MD is notified and patient

assessed

• Ensuring that all possible fall prevention protocols

are in place
