PBSGL in Canada � Canadian approach to CPD � Evolved from a single group of GPs at McMaster University in the mid ’80s � Now involves over 5000 practitioners in 600 groups in Canada (> 20% of GP population) � Foundation for Medical Practice Education (FMPE) � Used extensively in North America as part of “residency training” ... which is what we might call GP Training � International collaborations, including with Scotland PBSGL in Scotland � Piloted in Scotland 2003/04 (5 groups) � Rolled out in Scotland from Aug 2006 � Now involves ~2000 GPs in >180 groups (> 25% of the GP population in Scotland) and growing steadily. Increasing numbers elsewhere in the UK � Includes a small number of Practice Nurses and a growing number of Pharmacists � Rapid growth in GP Training from August 2009 What is PBSGL? � Practice based (i.e. based in clinical practice) � Small group (usually 4-9) � Learning � Peer-facilitated 2

  3. What is PBSGL? � Topic specific modules (adapted or ‘tartanised’ from Canadian material, then from SIGN/ NICE guidelines, now de-novo modules) � “The Gap” � Cases � Information Section � Commentaries & appendices � Commitment to change � But ... barriers to change 3

  4. PBSGL outwith GP/ Primary Care • Other professions – Pharmacy are particularly keen on this form of learning ... ?due to professional isolation. In addition, their use of small group education at present is very limited • Secondary Care Teams – “Sensitive discussions and decision making at End of Life” – successfully trialled ... but team hierarchy issues exist PBSGL Research � Quantitative and qualitative evaluation of GP pilot � Research into Practice Nurse (PN)-only and mixed GP & PN groups � Videoconferencing as a medium for PBSGL � Commitment & intention to change � Role of the facilitator and “what keeps them going” � PBSGL in GP Training � PBSGL in Pharmacy groups, and mixed GP & Pharmacy groups � Ten published papers to date in peer-reviewed journals, and more planned 4
