Health Center Program - Overview

For 50 years, HRSA-supported Health Centers have provided comprehensive, culturally competent, quality primary health care services to medically underserved communities and vulnerable populations.

Health Centers are community-based and patient-directed organizations that serve patient populations with limited access to health care.

Health Center New Access Points: $150M awarded to establish 236 new Health Centers access points.

  • Community Health Centers, Inc. - Glendale - to serve 3,910 patients by 2016
  • Wayne Community Health Centers - Escalante - to serve 2,085 patients by 2016

Health Center Mental Health Service Expansion – Behavioral Health Integration: $55M awarded to 221 Health Centers to establish or expand behavioral health services to over 450,000 patients nationwide.

Health Center Expanded Services Supplemental Funding (September): $300M in formula based supplements awarded to 1,195 Health Centers to help expand service hours, hire more medical providers and add oral health, behavioral health, pharmacy and vision services.

  • Family Healthcare: Cedar City (Co-located with SW Behavioral Health)

Health Center Expanded Services Supplemental Funding (September): $350M in formula based

  • supplements. Applicants must propose to use at least 80 percent of the Expanded Services funding

to increase capacity to provide one or more of the following eligible services: medical services; oral health services; behavioral health services; pharmacy services; and/or vision services. Up to 20% of funding may used to increase the availability of new and/or existing enabling services (e.g. care management/coordination, health education, translation, transportation, etc.)

**Other professional health visit can be visits with a broad range of health providers other than those listed under Table 5. Examples include: occupational therapists, Physical therapists, dieticians, nutritionist, podiatrist, speech therapist, acupuncturist, dental therapist, Naturopaths, acupuncturists, community health aides/practitioners, and chiropractors, etc.

Since 2012, Utah Health Centers have experienced a patient growth rate of 14%. Between 2012 to 2014 Utah Health Centers have expanded access to:

  • 9,052 uninsured patients;
  • 6,957 privately insured patients;
  • 2,248 Medicaid/CHIP patients;
  • 2,077 Medicare patients.
