Mental Health Referral Form

Rationale for project:

 Historically UCCN has only offered mental health

services to individuals who are HIV+ and are receiving medical care services at our clinics.

 As of December 1st, 2017, UCCN opened up its

mental health services to individuals who are NOT receiving medical care at UCCN. Therefore, we are streamlining the process to differentiate between the two groups.

Information on each improvement cycle:

 We informed our staff about the referral process

during our administration meetings.

 We informed our community collaborators of these

new services.

 We created a new Mental Health referral form.

Conclusions:

 UCCN implemented its new referral process on December 1st, 2017. As

  • f now, we have no new referrals.

Challenges:

 This is the first time UCCN accepts outside referrals.  Clients and organizations may not be aware of our services since it’s

been only a month since these new services were first offered.

 Our lack of referrals may have been strongly impacted by the inclement

weather and the holiday office closure period the last several weeks of December.

 Marketing/Word of mouth/electronic

communication

 Continue to remind staff and our collaborators

(case managers, doctors and medical staff, etc.) about our services and simplified referral process.

Referral process for someone who is not linked to the UCCM medical team:

 Completed referral form  Attached proof of HIV within the last 6 months

Appointment are made for the following clinics:

 Tuesdays at Uptown Clinic-845 W. Wilson- 9:00-1:00 PM

Thursdays at Northwest Side Clinic- 1612 N. Kedzie-9:00 AM-1:00 PM
