NON POWERED PRESSURE REDISTRIBUTION - DELIVERING QIPP

Overview: East Lancashire Hospitals NHS Trust traditionally used a conventional model of static foam mattresses and powered air mattresses to deliver its pressure area care provision. With escalating powered mattress usage costs, the challenge to achieve better pressure ulcer incidence rates and a complex product selection and availability process. The Trust sought an innovative solution.

Title: Installation of Mattress Replacement Systems

The Challenge:

  • Reduce pressure ulcer incidence through earlier prevention strategies
  • Streamline the existing processes to remove waste and become more

efficient

  • Contribute to Cost Improvement Programme objectives

The Solution: The paradigmsolution™ from KCI Medical using the

AtmosAir™ mattress replacement system. AtmosAir™ is a non-powered pressure redistribution surface with Self Adjusting Technology™ to maximise envelopment, immersion and minimise tissue interface pressures. Changing the product availability model and installing AtmosAir™ throughout the Trust immediately improves the standard of pressure area care for patients by providing an “active” surface for all. This initiative significantly reduces the number of mattress “upgrades” required and consequently eliminates the respective costs and unnecessary patient and equipment handling tasks associated with the conventional model. The provision of specialist high end equipment becomes the exception not the norm. Implementing the paradigmsolution™ brings a streamlined method of working, a better quality of mattresses for all patients, and a robust package of education and nursing staff support to achieve each of the objectives of the QIPP agenda. Quality – Proven clinical efficacy delivering positive outcomes for ALL patients. Innovation – Embracing new technology and a new operational model to improve and simplify product availability for ALL patients. Productivity – Releasing Time to Care by making mattress “upgrades” the exception rather than the rule. Eliminating unnecessary patient and equipment handling. Prevention – Providing high-risk mattresses for ALL patients regardless of risk assessment status. True prevention without selection or intervention.

The Future and Next Steps:

To continue to raise the level of education and awareness amongst staff regarding the role of the paradigmsolution™ in pressure ulcer prevention. Benchmarking and vigilance in the reduction of pressure ulcer prevalence and incidence rates. Continuous monitoring of powered mattress system usage. Comparison of key performance indicator status’ prior and post paradigmsolution™ implementation. Patient and nursing staff satisfaction monitoring.

Post Evaluation Notes & Comments

Clinicians

  • Physiotherapists commented

that they found it easier to mobilize and hoist patients out

  • f bed due to the stable

surface.

  • Decreased manual handling of

mattresses was also noted as being beneficial as Dynamic Mattress rental system requirements reduced.

Patients

  • No waiting times for patient

placement.

  • Patients rated the product as

being very comfortable.

slide-13

Cost Avoidance

Total Number of Dynamic Mattress rental days saved during the four week evaluation estimated as being 112 days. Assuming a daily rental price per day of £5 per product per day, the saving over the 4 week period would equate to £560 + VAT. Should this trend be replicated throughout the year then the saving for this one ward would equate to £7,280 + VAT

Contract Benefits

  • Reduction in patient risk of

developing pressure ulcers

  • Dynamic systems remain in

CCU & existing PU patients Only

  • reduced usage of dynamic

mattresses by 80 per cent

  • Ecological benefits of

introduction of non-powered solution

  • Enhanced patient comfort
  • Halving nurses time spent on

mattress changing releasing almost 2000 hours a year of time to care

  • £10,000 per year actual budget

savings plus cost avoidance methodology to monitor

  • Innovative contract solution
