How we identify young people with SEND and assess their needs …. Teacher assessment • Parents discuss concerns with • teachers and or SENDCo Concerns raised by members of • staff to the SENDCo Have hearing and vision been • checked? • Analysis of regular assessments of all pupils including meetings to discuss these each term • Make a referral to an outside agency
How does the school support my child to access the whole curriculum? How can my child be involved? • · Work with the Teacher and/or SENDCo to complete their Support Plans (where appropriate) for teachers to use when planning • · Complete a One Page Pupil Profile with teacher • · TAs will gather feedback regarding support and interventions • · Involvement in Annual Reviews • · Have regular access to talk with the SENDCo • · Pupils will set targets with teachers • · Talking with pupils about their strengths and their needs.
