House Staff Employees
− All House Staff employees are eligible for benefits except for Military House Staff
Eligible Dependents
− Spouse − Eligible Domestic Partner (same-sex or opposite-sex if you or your partner is age 62 and older) − Eligible Children up to age 26 (up to age 23 for Child Life Insurance)
Dependent Verification with Benefitsolver
− Submit required eligibility documents within 60 DAYS of Event Date − Dependents will not have coverage until Dependent Verification is completed but coverage will be retroactive to the event once documentation is received

New Hire Enrollment Period
− You have 31 DAYS from your Date of Hire (DOH) to enroll or waive your plans
Effective Date of Coverage
− Your Medical/Vision, Dental, Employee Assistance Program (EAP) benefits are effective on your DOH − All other benefits are effective the 1st of the month following DOH
New Hire Default Coverage
− If you do not take action, you will be assigned Employee-Only coverage Medical House Staff Aetna Choice POS II Plan Delta Dental PPO Basic Plan
Other Enrollment Periods
− Annual Open Enrollment in the Fall with changes effective Jan 1 the following year − Qualifying Life Event (QLE): mid-year life event such as Marriage, Birth, etc.
