Typical CASPER Requests

Most folks use CASPER Reports to verify their software

  • OASIS Agency Final Validation Report
  • OASIS Assessment Print
  • HHA Roster

Don’t forget to check the Review and Correct Reports for the new Quality Reporting Program measures (QRP/Impact Act)

slide-5
SLIDE 5

But Wait! There’s More!

Reports/HHA Maintenance/OASIS Quality Improvement And more options

  • Agency Patient-Related

Characteristics – Demographics

  • HHA Discharges
  • HHA Start Resume Care
slide-6
SLIDE 6

What Difference Does it Make?

You can run a HHA without checking any of these reports – the surveyors will never know Would you let your Granny be a patient? Would you let your child be an employee?

More CASPER Friends

Three main categories

  • Outcomes – Did the patient actually get better?
  • Potentially Avoidable Events – Oopsies! (PAE)
  • Process – How is the agency communication?

Each category has report types

  • Report – Graphs comparing national figures against

time points you specify, usually now and an earlier time point

  • Report–Text – Comparisons as above but in tables
  • Tally – Which specific patient are we talking about?

(PHI !!)

And What Does This Mean?

SLIDE 16

Tally Reports

Helps you know which specific individual’s assessment is triggering the measure you are interested in Again, make sure you run for the correct “interesting” timepoints A tally report for each category Calculations (numerators and denominators) at Downloads section at the bottom of the page, one file for each category https://www.cms.gov/Medicare/Quality-Initiatives-Patient- Assessment-Instruments/HomeHealthQualityInits/Home-Health- Quality-Measures.html

Transfer to P-D-S-A Cycle

slide-18
SLIDE 18

Checking on your PDSA

Depending on the report, some do not allow much resolution for the data

slide-19
SLIDE 19

Your Next PDSA Cycle

The most productive cycles are often very short

  • Single issue
  • Tallies for an afternoon
  • Meetings of less than 15 minutes

Don’t do a long cycle with an intent to “prove” the change concept until you are sure that the change is an improvement So CASPER reports are often left for long cycles or maintenance cycles or year-over-year checks

Stories

CASPER reports of stubborn “Improvement in Pain Interfering in Activity” led to increased OT involvement BTW – what category of measure is that? CASPER reports of unexciting “Diabetic Foot Care and Patient Education in Plan of Care” led to recruitment of podiatrist consultant BTW – category of measure is that?

More CASPER Friends

Many more reports possible than we covered today HHA Trend Analysis Reports for Outcomes measures provide very helpful run charts over time
