RFA Overview

Security Disability Income (SSDI) Outreach Access and Recovery (SOAR) Advocacy

  • Purpose: Identify Grantee(s) with plans to utilize the

SOAR methodology to work with clients, medical providers, and SSA/DDS staff to submit applications, provide follow-up, and ensure communication to improve outcomes and shorten process timeframe

  • Funding: Up to $500,000 for up to five providers
  • Award period: Sep. 28, 2018 to Sep. 27, 2019

  • Submit a minimum of 20 SSI/SSDI applications via SOAR

– 50% of the applications for DHS supportive housing clients

  • Submit the Appointed Representative form within 30 days of

accepting a client

  • 75% of the applications should be submitted within 55 days of

the protective filing date

  • Submit medical records/MSR within 30 days of claim transfer

from SSA to DDS

  • Use the SOAR Online Tracking Application for all SOAR

applications and document all relevant client information in HMIS
