- How we maintain
service users nutrition and hydration across the Trust.
Catering Services
Ensuring that all service users have:
- Enough to eat
- A choice of foods to suit all diets and appetites
– Dieticians – Range of menus – In right sort of packaging/containers
- Help with eating if needed
– Individualised care planning – Carer involvement – Protected meal times – Housekeepers – Volunteers
Nutrition & Hydration Care Pathway
- All service users are screened weighed, and assessed on admission
to identify risks of dehydration and malnutrition
- Nutrition care plans are produced for any patients shown to be at
nutritional risk and the plans continually evaluated and revised
- Weekly risk assessment for nutrition and hydration for all service users
- Access to Dietician and Speech & Language Therapist

- Senior Multi-professional Group to promote good physical

- Modern Matrons observation visits including weekly weight

- Nutrition group monitors all clinical incidents relating to food
and nutrition and follows each incident through to ensure suitable outcomes
