Name (Note gender/approximate age) • WHAT: What is the specific nature of the emergency/ request? • WHERE: Exactly where are you? Where are they? For lost person where LAST seen? • WHEN: When did contact occur? Lost person: Last seen where and when? Medical: Onset of symptoms, time of injury.

  28. CERT SKILLS utilization Ken Ow- Assisted a friend at church with treatment for bee sting reaction • and other issues, and called 911 and assisted him to and at the hospital. Trish Laut- Attended and stayed with accident victim until ambulance • arrived. Did anyone else use their CERT Skills this month? •
