MHSA Innovation (INN) Component Requirements MHSA Innovation Projects must contribute to learning Innovation (INN) Projects must do one of the following:  Introduce a new practice or approach to mental health system  Make a change to an existing practice in the field of mental health  Introduce a new application of a successful non-mental health promising community-driven practice/approach to the mental health system 2 SACRAMENTO COUNTY, DHS, BHS, MHSA MHSA STEERING COMMITTEE – 5/21/2020

INN 5 Project: Presenting Problem/Need  Forensic Behavioral Health Population o Individuals living with Serious Mental Illness o Multisystem involved o Jailed o Complex Needs  Presenting Problems: o Lack of collaboration among system partners, providers, and natural supports o Lack of immediate access to services/resources o Lack of discharge planning prior to release 6 SACRAMENTO COUNTY, DHS, BHS, MHSA MHSA STEERING COMMITTEE – 5/21/2020
