Background on ISCAN

  • Number of local reviews of services for children and young people with disabilities

took place in 2006

  • CYPP, Health, Social Care and Well-being Committee indicated their support to

develop an integrated multi-agency model of service delivery

  • Agreed to plan and implement a pilot project
  • Commissioned an independent consultant
  • Set up Project Board feeding into CYPP
  • Set up Implementation Group in November 2009
  • April 2010 ISCAN start date

ISCAN Front Door

  • Created a single “front door” for referrals for children with

identified/emerging additional needs/disabilities 0-19years

  • Single agency referrals / multi-agency referrals
  • Amalgamation of existing Child Development Team & ISCAN Panel
  • ISCAN Pathway for play assessments
  • Consider the request for a Lead Professional to co-ordinate services for

emerging or identified needs of the child and family and allocate as appropriate – 0-12 years

Aims of ISCAN – Lead Professional

To:-

  • Provide a “One stop shop”, accessible point of contact with

professionals

  • Alleviate stress/anxiety for children and families
  • Deliver an effective and trusted service for families and

professionals

  • Provide clear outcomes for children and families
  • Provide clarity of the pathway, endings and beginnings
  • Ensure better planning for transitions

ISCAN in Context

  • Education, Health, Social Care and Voluntary Sector services e.g. Action for Children

will work in partnership to plan and review assessment and interventions – using a Team Around the Family (TAF) model;

  • A lead professional will coordinate contact and meetings and provide a single friendly

point of communication for the family;

  • The TAF will plan actions and review outcomes with the child’s parents/Carers.

These will be recorded and monitored using the Family Support Plan

  • Age range is currently 0-12 years

Not just about service provision: – Changing the relationship between children, families and service providers – Changing cultures – Family Focused – Listening to families – Multi-agency/integrated way of working

ISCAN Pathway

  • Referral received
  • Within 7 days considered at ISCAN Panel
  • Allocation of Lead Professional
  • Using the Early Support Materials – family contact made
  • Within 15 working days of allocation – Lead Professional undertakes Initial

Assessment

  • Within 30 working days of allocation – TAF meeting takes places
  • Prior to TAF meeting – Lead Professional meets with family to complete Family

Service Plan

  • At TAF meeting – Family Service Plan Action Plan devised
  • Regular TAF meetings take place to review and monitor outcomes and set new

priorities

Outcomes

For Professionals:

  • Multi-agency team working has enable more complete information about a

child/family to be available.

  • Professional interface is better as a result the process of allocating support is faster
  • The TAF meetings in themselves are a learning tool – improving the understanding
  • f different professional roles
  • Although the “lead” professional provides responsibility for a case, it is shared and

supported by the TAF group and becomes a collaborative responsibility

  • More efficient delivery of services in a timely manner

  • Families feel more supported and less isolated
  • Friendly professional faces makes asking for help less daunting and more accessible
  • Takes away the extra strain of trying to co-ordinate the services your children and

family needs

  • One point of contact means you don’t have to keep repeating your story to many

professionals

  • Having a “Lead Professional” gives you more confidence in asking for support

knowing other professionals will feedback information to them

  • Allows you to concentrate more on your family and special needs child instead of

trying to arrange services
