PREMIUM PAYMENT AND CONDITIONS

PREMIUM PAYMENT

  • N6,000.00 per annum

OR

  • N500.00 per month for

12 consecutive months.

TERMS AND CONDITIONS

  • Individuals that are not LAPO MFB Clients

will REGISTER with the sum of N1,500.00 (N1,000 .00 for Account Opening and N500.00 for registration)

  • Individuals that already hold account with

LAPO MFB will only pay for registration.

  • A limit of liability of N100,000 per enrollee

per year

  • A maximum of 10 outpatient

consultations per year

  • A maximum of 15 days of hospitalization

per year.

  • For monthly payment (N500), defaulters

will not have access to Medical Care until

  • utstanding is paid and this is subjected to

waiting for another period of 3 months
