Concordia Disability and Survivor Plan Supporting You and Your Family During Pregnancy, Injury, Illness and Death
One Faith, One Community Concordia Disability & Survivor Plan (CDSP) Disability Benefits (and pregnancy) • The full cost of CDSP is paid by your employer • Disability Income – Benefit after 14 consecutive days – Pays 70% of your monthly salary to you to help replace lost income • Financial protection for you and your employer during extended disability: – Waiver of monthly contributions for employer – You get continued health coverage
One Faith, One Community Concordia Disability & Survivor Plan (CDSP) Survivor Benefits (similar to life insurance): • Should you die while actively working (not retired): – One-time lump sum death benefit paid to your named beneficiary: • 2 X annual salary • plus 1 X annual salary for each enrolled dependent child • up to a maximum of 6 X annual salary • For the death of an enrolled spouse and child: – A lump-sum death benefit of $10,000 – Active worker is the beneficiary
