  10. Practical/Hands-On Training Mockups Requiring Demonstration of HU Tools Such as: Procedure Use, Peer Checking, Phonetic Alphabet, & Flagging.

  11. HU Metric • Define Significant HU Event • Measure Improvement and Success by a Decline in the Significant HU Events and Rate Significant HU Event Rate = Number of Significant HU Events Over 18 Months per 10,000 Hours Worked (TVA & Contractor)

  12. Results to Date (Fossil Fleet) • Significant HU Event Rate = Number of Significant HU Events Over 18 Months per Human Performance Significant Event Rate 10,000 Hours Worked (TVA & Contractor) 0.1200 0.1100 0.1000 0.0900 0.0800 0.0700 0.0600 0.0500 0.0400 Data displayed is 18 month rolling average HU Rate

  13. HU Event Costs Cost of HU Events • The Challenge : Coal, Gas, PSS, and ESS FY2004 to Date Reduce TVA’s $60,000,000 Significant HU $50,000,000 Event Rate to $40,000,000 $30,000,000 meet Safety, $20,000,000 Performance, $10,000,000 and Cost Goals $0 FY12 FY04 FY05 FY06 FY07 FY08 FY09 FY10 FY11 to Date Cost of HU Events $36,821, $23,333, $20,172, $8,620,7 $38,409, $48,565, $48,979, $14,987, $642,966

  15. Challenges Our HU Tools Were Designed to Work Here

  16. Challenges How do HU Tools Need to Change to Work Here?

  17. Where We Are Now • Positive Results have Continued – Fewer Events this Fiscal Year v. Last • Methods and Results Shared with Industry via EPRI, INPO/WANO, NERC, HPRCT, and Other Industry Meetings • Gathering Industry Contacts and Benchmarking – Please stop by and let me know where you are in the process, and what you are doing. • Videos and Other Materials are Available • Continuing to Pursue Standardized Approach
