The case for change • Disconnected set of services (40+ providers of stop smoking, weight management, physical activity and alcohol services) little link to wider wellbeing agenda • Emphasis on driving individuals into services rather than supporting individuals to take greater autonomy and control • Service users and professionals found the system confusing • Good health is a top priority for residents
How have we changed? • A Healthy Staffordshire Hub - the provision of information advice and guidance. • Local community prevention programmes - that support healthy eating, increased uptake of physical activity and alcohol prevention • A Healthy Lifestyle Service – (Together 4 Health) offering additional support for lifestyle issues
Healthy Lifestyle Service • Together 4 Health • Provided by Staffordshire and Stoke on Trent Partnership Trust • Support is provided across the whole of Staffordshire • Offers tailored and structured motivational support / interventions • Offers up to 12 months support
