Hillsborough County Health Care Plan Success

At the time of losing his job and health care coverage, the resident was hospitalized for kidney failure. While hospitalized, Health Care Services assisted the resident with his Health Care Plan application. Six days after asking for assistance, Health Care Services was able to complete his enrollment into the Hillsborough County Health Care Plan. After being discharged from the hospital, he was able to continue his treatment through Health Care Plan providers and is now in stable condition. Since enrollment in the Health Care Plan, he has been approved for Social Security Disability.

SOAR Success

\ The resident was referred to Health Care Services for assistance through its community partner Florida Hospital Tampa. After enrolling her in the Health Care Plan, her case manager worked with her and submitted a Social Security claim for her in August 2018. Because of relationships and procedures put in place by Health Care Services, two months later the resident was eligible for benefits and had been eligible since February 2017. She received a back pay of $15,888 and monthly Social Security Disabilities Income benefits of $1,026. With the benefits, the residents was able to modify a room at her relative’s house and buy all the necessary medical equipment and supplies to live

  • comfortably. She also now receives in-home nursing assistance and physical

therapy. A Hillsborough County resident, who was a kidney-transplant recipient and suffered from chronic kidney problems, came to Health Care Services for assistance after he lost his job and his health care coverage. A Hillsborough County resident was involved in a major car accident in 2017 that left her legs, hip, and head severely

  • injured. Because of the accident and

severity of injuries, she lost her job, her apartment, and her car. She moved into a relative’s home and lived on the couch while going through physical therapy and dealing with unexpected infections.
