Lead in Blood

  • 90% is bound to RBC
  • 3% is in Serum
  • Chronic effects arise where the serum levels are

elevated (Lead is released to the CNS)

  • Iron and Calcium sites are the primary entry

routes

Lead Deposits in the Respiratory Tract

  • Less than 5 microns- Deep Lung penetration-

95% will be absorbed

  • 5-120 microns- Cleared by Mucociliary action
  • 120 microns- Nasal deposits- expelled or

swallowed

GI Absorption

  • Most absorbed Lead in the system is through the

GI tract

  • 10% is absorbed
  • 4-10 times higher Lead absorption with an

empty stomach

  • Increased Calcium in the diet= decreased Lead

absorption

  • Fiber binds Lead and decreases absorption
  • Skin absorption is minimal

SLIDE 8

Dietary Habits

  • Increase number of meals per day can decrease

Lead uptake

  • High Fiber
  • Low Fat
  • Foods high in Iron and Calcium

slide-9
SLIDE 9

Lead Excretion

  • Kidney (Keep in mind that Blood Lead should

rapidly decrease during pregnancy due to doubled filtration during pregnancy)

  • Colon
  • Minimal biliary excretion, Alcohol use may affect
  • Whatever is not excreted is stored in bone

ZPP

  • May be affected by inflammatory conditions
  • Diet low in Zinc
  • Some meds may affect
  • Normal value is less than 100mcg/100g
  • Blood Lead may change drastically but ZPP is

slow

Medical Conditions to Defer from a High Lead area

  • Severe Anemia
  • Liver Damage
  • IDDM with Renal Compromise
  • One Kidney or on Dialysis
  • Current Blood Lead above 30

11

slide-12
SLIDE 12

Chelation

  • Not approved by the FDA for the reduction of

Blood Lead

  • May cause Kidney failure from rapid movement
  • f fluid and particles through the kidney

12

Low Level Health Impacts

  • All Physicals done at Pb level equivalent to the

voluntary reduction value of the time.

  • No value or benefit seen from review of a

sample of this population of workers

13

Value of Baseline Medical Evaluations

  • Compliance with standard
  • Only way to protect yourself- how do you

understand where you are if you don’t know where you started from?

  • If you work for Troy Greiss, you can never have

enough data!!!
