P- Prepare and Plan Has a list been prepared of the tasks that have potential to cause a hand injury? Eg; Pinching, lacerations, crushing, struck by object, lifting or sudden/unexpected movement. 2. I- Implemented controls Are risk controls for the tasks identified being implemented? What are the controls? Eg; Correct gloves, machine guarding, correct equipment and tools, assistance from others 3. N- Never put your hands in the Line of Fire Is there potential for the workers hand/s to be crushed/pinched/lacerated/impacted? Eg; Are hands placed on top of/ between the load and a fixed object? is machinery guarding in place? correct tools used? Visibility of work area? 4. C- Communication Has there been discussion about the task, risk controls and potential for hand injury? Eg; Take 5, Prestart, Re-start? 5. H- Housekeeping Is the site, work area clean and organised? Eg; Fence footings stacked evenly, slips, trips and fall hazards eliminated. Comments/Further actions 17
