Grant Seekers’ Technical Assistance Meeting 2020 SECTION A: AGENCY INFORMATION (Maximum of 3 pages for sections A & B)

A1. What is your agency’s mission? A2. Summarize your agency’s qualifications, experience, and expertise relevant to the program seeking funding (e.g. years of operation, accreditations, experience of staff and volunteers, etc.). A3. What are your main reasons for seeking United Way support at this time? How do you hope to benefit from the partnership? A4. In what ways did your agency collaborate with United Way in the past year? A5. In what ways does your agency collaborate with other agencies to leverage resources and/or enhance services? A6. If you experienced any substantial changes within your agency during the past year, please explain. If you anticipate any substantial changes within your agency in the coming year, please explain.

We know questions here are similar to questions on the RFQ application. Why are we asking again? Because reviewers will only have access to the 1-year proposal, not your RFQ. Feel free to cut and paste RFQ information and elaborate where necessary.

B1. What services will the program provide? B2. What is the target population and geographic area of the program? How many people are served? What portion of recipients enter services at or below the ALICE threshold? B3. Identify the community need(s) the program addresses? In what ways will clients benefit from program services? B4. If applicable, how does the program serve diverse populations? B5. If you experienced any substantial changes within your United Way-funded program during the past year, please explain. If you anticipate any substantial changes in this area during the coming year, please explain.

Again, we know these questions are similar to those on the RFQ. Feel free to cut and paste RFQ information and elaborate as needed. Section C: Funding priorities

C1. Please describe how your program aligns with the selected subcategory and how it will support the respective funding priority. Cite evidence to support your narrative: (maximum of 1.5 pages)

We’ve also given you a bit more space to answer C1 compared to the RFQ. So let ‘er rip!

Grant Seekers’ Technical Assistance Meeting 2020 SECTION D: LOGIC MODEL (Maximum of 2 pages)

The logic model should reflect your resources, activities, outputs, and the outcomes for the year of funding.

The logic model should look familiar as you have completed this table in the past. It’s a nice way to capture the main details of your written narrative for a holistic view of your program. It gives you a place to take view your inputs, indicators and measurables, through the impact of your program. Section E: In lieu of having existing partner agencies currently receiving United Way funding

submit a separate year-end report, section E will capture the information.

Please report data from the current funding cycle as well as what you will collect during the upcoming grant cycle. For some of you, this will likely be the same as what you are currently collecting and reporting to us. We have provided two tables in the event you have decided to change the key

  • utcome and indicators moving forward.

F3. How did you determine the amount of your United Way request? What costs and other factors affected your decision to request this amount?

Please fill out the required supplemental Excel budget form for your program and be as thorough as possible so that the funding request is in line with program services and expenses.
