VA Provided Institutional Care Community Living Center (Vancouver) • 72 beds  Skilled Nursing 28-30 beds  Hospice/Palliative 12-15 beds  Long-Stay 7-8 beds  Rehabilitation 20-22 beds • Eligibility – VA is authorized to provide nursing home care to all Veterans who need such care and who meet the minimum length of service requirement – VA is mandated to provide nursing home care to Veterans who need such care and at least one of the following: • Who have a service-connected (SC) disability rated at >70% + • Based on SC- disability or un-employability • Those whose need for such care is for a SC-disability

  6. Purchased Institutional Care Contract Nursing Home • 40-45 Contracted Facilities • Average Census – 100 • Eligibility  Any Veteran in need of such care (Nursing home level of care) for a Service-Connected disability  Any Veteran who is in need of such care (Nursing home level of care) and who has a Service-Connected disability rated at 70% or more State Veterans Home VAPORHCS provides oversight and annual inspection •  The Dalles (151 beds)  Lebanon (154 beds) Veterans must be in need of nursing home level of care •

  7. VA Provided Non-Institutional Care HBPC: HBPC is a comprehensive, longitudinal and interdisciplinary primary care delivered in Veteran’s homes. Veterans who would especially benefit from HBPC are those at increased risk for hospitalization, nursing home placement, or emergency room utilization in the absence of the service. • No Service Connected requirement • 4 Teams in Portland/Vancouver, North Coast, Salem/Newport/Grand Ronde, Bend/Warm Springs • Interdisciplinary Teams: MD/NP, Nursing, Social Work, Psychology, Dietitian, OT/PT, Pharmacy, Program Support • Electronic consult and MD order

  8. VA Provided Non-Institutional Care Geriatric Evaluation Clinic: • Comprehensive assessment with goals of reducing nursing home placements • Process not a program • Interdisciplinary Plan of Care • May be provided in a variety of settings  Inpatient or Outpatient Geriatric Evaluation and Management Clinic  Geriatric Primary Care (Geri PACT) Clinic  Geriatric Problem-Focused Clinic  HBPC  Community Living Center

  9. Purchased Non-Institutional Care • Purchased Skilled Home Health • Home Hospice • Home Infusion • Homemaker Home Health Aide (H/HHA) • Outpatient Respite • Veteran Directed Home & Community Based Services (VD- HCBS) • Contract Adult Day Health Care

  10. Purchased Skilled Home Care PSHC: VA provider-ordered, in-home skilled services “home health” • No Service Connected requirement • Community agencies provide services • Must meet Medicare criteria: homebound • Have “skilled” needs for: Nursing, PT, OT, ST, MSW • Generally short term, focused care • Electronic consult and MD order (requires VA PCP or Specialty MD to follow episode of care) • Requires face to face visits with provider within last 30 days

  11. Home Hospice Services Hospice is comfort-oriented and supportive services for Veterans with advanced life-limiting disease. • No Service Connected requirement • Community agencies provide services • Must meet Medicare end of life criteria • Nursing, social work, therapy, chaplain, bath aide, volunteer services • Emphasis is on comprehensive management of the physical, psychological, emotional, social, and spiritual needs of the Veteran • Electronic consult and MD order

  12. Purchased Care – Non-Skilled Services • Veterans must meet the following eligibility criteria: – 3 or more ADL dependencies (eating, bathing, dressing, toileting or transfers), or – Significant cognitive impairment, or – Require services as adjunct care to community hospice services OR – 2 or more ADL dependencies, AND two or more of the following conditions: • Dependency in 3 or more IADLs (meal prep, shopping, money management, housework, etc.) • 75 years or older • Lives alone in the community • Clinical depression • Recently discharged from a nursing facility • Medical High utilizer (3+ hospitalizations or 12+ clinic/ECU visits/yr)

  13. Homemaker/Home Health Aide H/HHA is personal care and related support services provided in the home to frail or disabled Veterans who would otherwise require institutionalization. • No Service Connected criteria • Community agencies provide services • Need to be nursing home level of care • Focus of care is on ADLs • Standardized assessment tool to determine hours/week • Electronic consult and MD order (requires VA PCP) • Prioritize end of life care, discharge from inpatient setting

  14. Outpatient Respite Respite is time-limited care provided for the purpose of helping the Veteran to continue residing primarily at home. Respite providers temporarily replace the caregivers to provide services ranging from supervision to skilled care needs. • No Service Connected criteria • H/HHA agencies provide services • Need to be nursing home level of care • Electronic consult and MD order (requires VA PCP) • 30 days per calendar year (all respite)

  15. Contract Adult Day Health Care Adult Day Health Care is a therapeutically oriented outpatient program offering health maintenance, rehabilitative services, socialization, and caregiver respite and support. • No Service Connected requirement • Contract with 4-5 centers (Portland, Vancouver, Corvallis) • Need to be nursing home level of care • Prioritize Veterans at end of life or discharging from inpatient • Generally 2 days/wk. • Electronic consult and MD order (requires VA PCP)

  16. Veteran Directed HCBS VD-HCBS VHA collaborates with State Area Agency on Aging to provide Veterans at risk of nursing home placement with a flexible array of Veteran-selected in- home services for maintaining Veterans in their homes. • No Service Connected requirement • Collaborative with Multnomah, Washington, and Clark County Aging Services with plans to expand to Clackamas and Marion • Need to be nursing home level of care • Veteran receives monthly budget for approved plan of care • Electronic consult and MD order (requires VA PCP)
