WHAT WE DO: A DAY IN THE LIFE OF AN FCP

MSK Triage phonecalls 20 minute appointments: assessment, diagnosis, explanation, investigations, initial treatment and self- management advice, onward referral. Registering patients for University of Keele Qualitative FCP Study (Finished Jan) Completing National Standardised Data Template for each patient Completing Outcome Data for each patient

WHAT WE DO: PATIENT TRIAGE

• Patient calls GP practice
• Speaks to a care navigator, who

refers to the flowchart

• If they meet criteria offered a same

day appointment with APP.

• Patient calls GP practice.
• Booked GP appt for MSK problem
• GP sees patient and books them APP

appt and/or orders investigation and/or advises them to come back to GP for follow-up

• Patient added to triage list for ANP
• r GP.
• Triage clinicians uses flowchart. If fits
