Diabetes Continuum

Vision

Provide a comprehensive solution that supports all associates and their families

  • n their well-being journey to

prevent and delay diabetes. And, if they already face the challenges of this disease, the solution will provide unfailing support to limit its progression.

An early focus on Prediabetes

The Impact of Diabetes Prevention remains Profound

  • Health care costs: For every case of diabetes prevention,

average health costs are reduced by $5,000 per year in 2016 to a projected $15,000 in 2025

  • Type 2 Diabetes prevention: Out of 5,000 participants,

hundreds of cases of type-2 diabetes will be avoided through weight-loss and lifestyle changes for a potential annual cost avoidance of $2 million1

Humana’s DPP approach improves

  • utcomes

Humana’s DPP is successful:

  • 100% of eligible population have been invited to join
  • Over 5,500 participants to date (~25%)
  • Strong engagement: 90+% continue through 4+ weeks and 67% complete the program
  • Outcomes at the clinically significant level (average weight loss in the 3-4% range)
  • Participants PMPM trend 50% below that of matched non-participant peers1
  • RX costs increase rates were also below matched non-participant peers

However, sustaining weight loss is an ongoing challenge.

  • 1. Based on Humana associate claims analysis from 2014-2017

2. Cohort is ~13,000 associates tracked from 2012 to 2014 who have biometric ratings in each year. (n~13,000)

Key learnings are helping us evolve and scale impact

  • Program success is directly tied to the degree of engagement
  • Useful to have different modalities and multiple opportunities for associates to connect
  • Overall weight loss is similar across all DPP modalities; however the cost is not
  • Food tracking and social connection are key elements of successful weight loss
  • The word “Diabetes” is a significant barrier. Associates assumed DPP was for people with

diabetes and struggle to internalize “at-risk” references.

  • Sustainability is key; greater structure needed to guide participants to what’s next
  • Short term interventions could help those not yet ready to commit to 1 year
