What is the My Health Record System

Timing Content Discussion

Slide 5: My Health Record system

My Health Record System

The My Health Record system is a way of sharing important health information between Health Professionals and Health Organisations. The My Health Record system is an ‘opt-in’ system where a patient can choose to store their health information. Slide 6: My Health Record system [cont.]

My Health Record System [cont.]

The My Health Record system is safe and secure, it can only be accessed by professionals who have your consent to do so, it is easy to see which organisation has looked at a record, it is easy to change permissions or remove documents and it is very secure – so it’s private and confidential. Slide 7: My Health Record system [cont.]

My Health Record System [cont.]

The My Health Record system will allow information to travel with you, wherever you go and will allow access and sharing of important health information with health professionals wherever it is needed.

Slide 10: Council of Australian Governments [COAG]

Council of Australian Governments [COAG]

To monitor change, the Council of Australian Governments [COAG] has set measurable targets to monitor improvements in the health and wellbeing of the Indigenous population. These targets focus on:

  • health
  • housing
  • early childhood
  • education
  • economic participation and
  • remote service delivery

These targets are underpinned by building blocks. Slide 11: Target Early Childhood

Target Early Childhood

Young Indigenous children need equal opportunities to learn, develop and socialise. They need access to quality early childhood education and care services, along with child care and family support services [such as parenting programs and supports] They also need access to appropriate facilities and physical infrastructure, which include, sustainable early childhood education and health workforce learning frameworks along with

  • pportunities for parental engagement.

Slide 12: Target Health

Target Health

Indigenous access to adequate, preventive and comprehensive primary health care is essential to, reduce excess death rates, close the gaps in early childhood mortality and life expectancy. COAG commitments in this area recognise the importance of all parts of the health sector. Key components of health commitments include the responsiveness of, and accountability for, achieving government and community health priorities. The health building blocks, COAG focuses on include promotion

  • f healthy lifestyles at all ages and related management and

treatment of chronic diseases.

Slide 13: Governance and Leadership

Governance and Leadership

Strong governance is essential for sustainable and effective

  • utcomes for communities.

In this context, Indigenous communities need to be engaged in the development of reforms within the Closing the Gap framework. This building block recognises the importance of skill development for Indigenous people to:

  • exercise their rights and responsibilities
  • take effective control over the development and

implementation of policies that affect their lives

My Health Record system and CTG

This section provides an overview of My Health Record system and the supportive initiatives that are needed to ‘close the gap’. Slide 15: My Health Record system and CTG Initiatives

My Health Record system and CTG Initiatives

The My Health Record system along with appropriately supported initiatives could be an extremely useful tool as part of the Close the Gap initiative. The My Health Record system has the ability to:

  • involve young people in their own health earlier by

encouraging them to use the My Health Record system

  • enable greater consumer control of health giving them

empowerment and enhancing self determination

  • support better collaboration between health professionals,

consumers and their families in the provision of health care

  • identify consumers who require intervention earlier due to the

sharing of important health information

  • enable remote and rural health services to gain access to

improved health information, meaning more informed decisions and interventions earlier

Slide 16: Benefits of eHealth

Benefits of eHealth

This section provides an overview of eHealth benefits to key stakeholders including:

  • Organisations
  • Aboriginal Health Workers and other Clinicians
  • Service boards and community councils

Slide 17: Organisations

Organisations

Over time, the My Health Record system will provide

  • rganisations with more information about their patients.

The My Health Record system will help provide better care, save time, help reduce adverse events and help organisations become better connected to other healthcare services Slide 18: Organisations [cont.]

Organisations [cont.]

The My Health Record system can help reduce the amount of time spent searching for health information such as, specialist letters, discharge summaries, prescribed medicines and test results. This can help improve the efficiency of your practice. Slide 19: Organisations [cont.]

Organisations [cont.]

The My Health Record system can help improve the health information your clinicians have available at the point of care. It can also help clinicians gain a greater understanding of the patients’ needs and health issues. This will help improve the patients’ experience and also help clinicians provide high quality and safe healthcare appropriate to the patients’ needs. Slide 20: Organisations [cont.]

Organisations [cont.]

Participation in the My Health Record system is incentivised. This means there is an opportunity to claim from the eHealth Practice Incentive Program [ePIP] and the Medicare Benefits Scheme [MBS] for eHealth based consultations. Adopting the My Health Record system will help your practice withContinuing Quality Improvement and improved Data Quality that will support accreditation.

Slide 21: Health Professionals

Health Professionals

All Health professionals who use the My Health Record system can benefit. Especially those who provide services to newborns, young children and new mums, those who work in remote locations and work in locum roles and are not familiar with the community and community members. Slide 22: Health Professionals [cont.]

Health Professionals [cont.]

For Aboriginal Health workers and Clinicians, who work in remote or isolated areas, the benefits of the My Health Record system will make it easier to collaborate with other health professionals and allow them to share information with the wider team electronically [with the right software]. Slide 23: Aboriginal Health workers and other Clinicians [cont.]

Health Professionals [cont.]

For Aboriginal Health workers and Clinicians, who provide services to newborns, young children and new mums, the benefits of the My Health Record system will include:

  • access to the Australian Immunisation Register to review

existing immunisations

  • joining-up services making it easier to spot potential issues

earlier and ensure that newborns and children that require further help are referred earlier and updating the My Health Record with details of the consultation with an event summary so other health professionals can easily access the information. Slide 24: Participating in the My Health Record system

Participating in the My Health Record system

Participating in The My Health Record system can;

  • f Appropriate Care

Improved Health Information Enables Increased Delivery of Appropriate Care

Health professionals have access to more health information than they may have otherwise had before. This additional information can:

  • help inform decision making
  • support safer health care
  • culturally appropriate
  • decision Support Tools

better application of clinical guidelines and best practice. Slide 28: Improved Capability to Apply Best Practice and Undertake Preventative Care

Improved Capability to Apply Best Practice and Undertake Preventative Care

The My Health Record system supports the continuity of care. Using My Health Record system ACCHS can help improve the application of best practice treatment, screening and prevention for Aboriginal and Torres Strait Islanders people through improved:

screened increase participation in screening programmes

  • initiate preventative measures in response to risk

factors and allow other providers to contribute to the management and success of these interventions

  • Shared information on prevention methods to help
