Program Goals / Mission

§ Provide personalized restorative care to seriously

wounded, ill and injured Airmen, caregivers and families

§ Provide support throughout transformation “back to

duty”, separation or retirement as an “Airman for Life” well-equipped to manage challenges regardless of injury

  • r illness

§ We are Wounded Warrior Advocates!!

AFW2 Program Eligibility

n Very Seriously, Seriously wounded, ill or injured on the casualty report

  • r by DoD Medical Authority

n Airmen with highly complex medical conditions (service connected or in

  • the-line of duty) confirmed by a Medical Authority (examples: tick-

borne Illnesses, cancer, invisible wounds, chemical exposure) (final approval authority: DPFW)

n Airmen diagnosed w/ service connected or in-the-line of duty PTSD, TBI

  • r MST, (verified by medical authority) and are under consideration or

referred to MEB

n Airmen in training status (including BMTS), separated from their

  • rganization with complex medical conditions on case-by-case basis

n Purple Heart Recipients n Air Reserve Components (ARC): Same criteria as above to include

validation that injury/illness was service connected and in-the-line of duty and/or while on active orders, or on medical continuation orders

n Integral part of the Recovery Coordination Program n Partner to provide support and care management n Key Care Management Team Members: n Airman / Caregiver / Family n Airman’s Commander n Airman’s Supervisor / Co-workers n Airman’s First Sergeant n Medical Team n AFW2 Care Team

Recovery Coordination Pgm (RCP) Triad

n Boots on Ground - RCCs n Located at 42 locations / regional coverage n Identify Airman / Caregiver / Family needs and

establishes goals

n Coordinate medical and non-medical services n Reach back – Region Team Lead & NMCM SMEs at

AFPC

n Direct access to Personnel program SMEs n Allows for continuity of care n Support Network – Recovering Airmen remains

assigned to their Squadron

n Promotes the “We Take Care of Our Own” ethos

■ 3,470 = Active population (1,991 AD / 1,479 TDRL) ■ 2,896 Regular (83%) ■ 402 Guard (12%) ■ 172 Reserve ( 5%) ■ Support 5,650 veterans on as-needed basis ■ AFW2 Case Category ■ 809 Illnesses ■ 1,827 Invisible wounds (1,265 PTSD) ■ 834 Physical wounds (includes TBI) ■ Combat vs. Non-Combat Enrollment ■ Combat Related = 14% ■ Non-Combat Related = 86% ■ Enroll ~110 new Airmen per month
