Shipley and Westcliffe Medical Practices
A new chapter
Affinity Care
Background
- Shipley 8,400 patients
- Westcliffe 11,400 patients
- Decision taken last July to merge the patient lists
- We have a strong history of working together and
since 2014 we have worked under the same Partnership
- Currently we have duplication of many functions
across both sites and there are opportunities to work closer together
- Single (merged) list is an enabler of this
Affinity Care
National drivers
- Clear push for larger practices nationally with
new contract and Primary Care Networks
- Desire to see a wider team of people working
in GP – pharmacists, physiotherapists, community workers, advanced nurse practitioners, paramedics and more
- New models of care
- New ways to access care

you based within our own practices
Affinity Care
When is it happening?
- Monday 15th April 2019
– When our clinical system provider will start to merge the two clinical systems together – We will close normal clinics at 3pm to allow this – Emergency cover will be in place via the practice telephone system
Affinity Care
When is it happening?
- Tuesday 16th April 2019
– We will reopen at 8:00am as normal – You will still be able to ring us using the normal practice telephone numbers – We have on-site technical support planned for that day – Essentially, it’s “Business as Usual” !!

Care Navigation
West Wakefield Model
Affinity Care
”A person-centred approach that uses signposting and information to help primary care patients and their carers move through the health and social care system as smoothly as possible to ensure that unmet needs are met”
Care Navigation
Affinity Care
- GP forward view 2016 - widening workforce.
- Care Navigators will be pivotal to navigate to the
extended primary care team and local services
- Patients receive the right care, by the right professional,
at the right time and in the right place (locality)
- This means one visit, not two or three or more for the
patient
- Improved job satisfaction for frontline staff
