SCHOOL VISIT CHECKLIST- ONSITE DIRECTOR

This form is to be as a tool when you are conducting an onsite visit. Also to enhance the communication with each of the teams. Instructions: Please fill this out for every site that is visited. Prior to leaving the school site(s), please fax a copy to your Ops & H&N team member. This does not take the place of the Onsite Report that gets typed up. District and Site Name: Date of Visit: Reviewer’s Name: Meal Reviewed: Production Records, Standardized Recipes, and Menu Meets Standards Needs Improvement Production records match ALL foods being served · Serving Line, Salad Bar(s), Milk Serving Sizes match production records and/or recipes · Check scoop sizes prior meal service · Refer to recipes for assembly instructions Standardized Recipes are being used correctly · Recipes are pulled prior to production · Check that ingredients used matches recipe Recipe books are current and available for use in the kitchen Menu posted matches production records Production records are being filled out correctly and complete Menu matches food production records match tray line Select a weeks’ worth of food production records to review for accuracy Reimbursable Meal and Meal Service SFE Reimbursable meal form is at every register or POS, and filled
