  5. 5 ACCREDITATION Accreditation Canada is an independent, not-for-profjt organization that accredits more than 1,100 health care organizations and more than 6,000 community and facility based sites. Venta Care Centre will be undergoing accreditation in February, 2018 Accreditation through the Qmentum Program is an ongoing process of assessing health care organizations, such as Venta Care Centre, to identify what is being done well and what needs to be improved. Venta work with Accreditation Canada to help improve quality, safety and effjciency so that we can provide the best possible care and service to our residents and families. Qmentum standards guide quality improvement efgorts to achieve safe and efgective heath services. Standards are grouped into four main categories: 1. Leadership 2. Medication Management 3. Long Term Care 4. Infection Prevention and Control ACCREDITATION CANADA

  6. 6 Within each standard are a number of ROP’s – Required Organizational Practices – which are essential practices or systems Venta Care Centre must have in place to ensure safety and minimize risks. Some examples of these ROP’s include, Resident Safety Management and resident safety training and education. An essential ingredient to high quality healthcare is actively including the voice of the residents and families to improve quality, safety and health outcomes. Engaging residents is critical to understanding their experience and can lead to changes in practice, service delivery and outcomes providing a true resident centered experience. Venta Care Centre follows the Resident and Family Centered Care approach that fosters respectful, compassionate, culturally appropriate and competent care that is responsive to the needs, values, beliefs and preferences of all residents and their family members. This approach shifts doing something to or for the Resident / Family to do something with the Resident/ Family so that that a true partnership can form between the health care provider and the Resident. We will be holding regular Resident / Family Advisory Group Committee meetings in order to better engage Residents and Families in sharing their insight and information about their experience in ways that the health care team can learn from.   7. 7 Some important initiatives we would like to review with you: • Complaint / Concerns Process • Venta Care Centre has a complaints process in place to ensure that resident and family complaints or concerns are given priority attention to ensure appropriate and timely intervention and resolution. • Privacy and Confjdentiality • Resident Safety Management Process and Disclosure • The facility has multiple resident safety initiatives in place that include Falls Prevention, Appropriate Use of Antipsychotics and Wound Management. • Community partners • Our partners with the community include our Residents and their Families, Alberta Health Services, Remedy’s Pharmacy, Alberta Health and Wellness, Continuing Care Safety Association, etc. • Accountability • Venta Care Centre is accountable to a number of agencies that have set standards in place we must be in compliance with. This includes the Long Term Care Accommodation Standards (through the Alberta Government), Alberta Health Services Continuing Care Health Services Standards and Accreditation Canada Standards.   16. 9 What is PUPP? • PUPP = Pressure ulcer prevention program • Weekly wound assessments completed • Consultation with AHS wound care nurses • Interdisciplinary biweekly meetings to review statistics, trends and discuss complex cases to ensure a reduction of wounds

  20. 13 Research indicates that hearing loss is associated with: embarrassment, irritability, stress, anger, withdrawal from social situations, depression, danger to personal safety, reduced general health, loneliness, isolation, paranoia, reduced coping skills, and reduced overall psychological health.

  22. 15 • What medications were good then might not be the best choice now. • Deprescribing is the planned process of reducing or stopping medications that may no longer be of benefjt or may be causing harm. • The goal is to reduce medication burden or harm while improving quality of life. • The team works with the NP, Dr, pharmacist, unit nurses and care aides

  34. 9 Catch a Falling Apple Program • Falls Risk Management Program • According to AHS, Fall-related injury was the fourth leading cause of injury death in 2010 • The % of falls among adults over the age of 65 is estimated to be 33% • This fjgure increased to 50% for those over the age of 80 • Falls are the most common cause of injury in older adults • The most serious impact of falls is death. Between 2008 and 2010, older adults accounted for 80% of fall related deaths (Alberta Centre for Injury Control & Research, 2012)

  36. 11 Prevent a Fall before it happens Most falls are preventable All members of the health care team and family members have important roles in preventing resident having a fall or reduce the possible injuries related to falling which may include: • Falls risk assessments are completed at the time of admission • Ensure falls prevention strategies are implemented after assessment • Monitor resident’s status and communicate changes to the rest of the interdisciplinary team • Assess equipment needs related to safe resident handling including needs for purchase, repair and adjustment.

  37. 12 Catch a Falling Apple Program • Goal is to Reduce Injury • The Symbol of a Red Apple is used to identify a frequent faller in the facility • A resident who had 5 or more falls in a calendar year will get a Red Apple • It is a visual reminders
