the CT Medical Home Initiative providing support to families and professionals.

 We have 10 staff that do specialty coordination

related to;

  • Early Dental Care
  • Medical Home
  • Transition
  • Autism Spectrum Disorder
  • Deaf/Hard of Hearing

  • Autism Specific Groups
  • Apraxia Support Group
  • Open Play/Support
