Team goals have been modified based

Focus on improving the health and health care

  • utcomes of 50 residents with chronic conditions

served by 15 fire stations covering Osceola County.

to appropriate, comprehensive health care and health supports, to include the necessary chronic disease specialty.

that improve overall health status and fiscal health of the system.

Here are the next steps for this project.

Hire the patient navigator to be located with public safety to assess identified resident issues and connect those residents with community supports.
