Self-Determination: A way of life

An individual’s basic right to decide how to live her/his own life and be a full citizen.

Self-Direction: A tool to support a self-determined life

Maximizes choice and control for people who use long-term care services and supports. Values the role individuals and families play in knowing what works best for them. Promotes community connections, including employment, as a way for individuals to create valued roles and use unpaid and natural supports.

Working with People and Their Supports

Identifying Self- Directed Supports

  • The person partners with their Consultant to

identify trusted people or agencies to provide support.

  • These may be people or agencies the person

knows already or that they discover with their Consultant. Arranging Services and Payment

  • The person determines where, when, and how

services will be provided, as well as the rate to be paid for supports.

  • The person partners with their Consultant to

create a plan, and hire and pay their supports with the help of their Fiscal Employer Agent (FEA).

  • The person uses their budget authority to direct

their service plan.
