Supervised consumption data

  • Supervised consumption is when a service user receives their opiate substitution

prescribed medication (methadone / buprenorphine / espranor) which they are required to take on site of the pharmacy

  • For some service users, they are able to take their medication home to self-administer
  • CGL make the appropriate clinical decision on what the service user requires
  • 36% of those prescribed as of December 2018 (125 out of 351 clients) utilised the

supervised consumption offer

SLA’s and competence

  • 26 pharmacies have a Service Level Agreement (SLA) to offer supervised consumption
  • The SLA stipulates each pharmacy taking part in offering / delivering supervised

consumption has to attend training

  • There also needs to be a named lead who has an ‘in date’ DoC (Declaration of

Competence)

  • Going forward we require you to complete an enrolment on PharmOutcomes to deliver

supervised consumption which is linked to your DoC, this should be renewed every 2 years. You will only need to enrol once and PharmOutcomes will hold this information, rather than you having to send your DoC’s into us. Once a DoC expires, you will be given 3 months grace to complete a new one

  • Failure to comply can result in being removed from the approved list that WBC hold and

CGL use to allocate clients

The SLA’s stipulate that a named lead pharmacist at each branch must be competent to deliver these services so they must complete a declaration of competence: Supervised Consumption of Prescribed Medicines Service This will be renewed every 2 years

Missed pickups (from CGL Pharmacy Guidance) When a service user ‘misses a pickup’ this could be an early warning sign that they are experiencing difficulties e.g. illicit use. It is essential we are informed within 24 hours

  • f a missed pickup so that we can identify additional support requirements.

The process is: Pharmacies should submit ‘notifications of missed collections’ via PharmOutcomes for those receiving supervised or unsupervised doses. This should ideally be completed on the day of the missed pickup, so that actions required can be agreed by CGL the following morning, or within 24 hours. It is essential that this action is completed for those on less frequent collections. e.g. if a service user failed to collect medication on 01/04, a notification should be submitted each day of the missed collection, so on 01/04, 02/04 and 03/04 if they continue to miss

SLA states:

3:1 To ensure compliance with the agreed treatment plan by:

  • Notify Warrington Drug and Alcohol Services (known locally as CGL or Pathways to Recovery) of

failure to attend or other non compliance within 24 hours (1 working day) to ameliorate risk

  • Notify Warrington Drug and Alcohol Services (known locally as CGL or Pathways to Recovery) with

concerns over well being

  • Risk management means it is vital that all pharmacies alert CGL Pathways on day 1,

day 2 and day 3 with regard to service users failing to pick up their prescription/s CGL Guidance states :

Three Consecutive Missed Prescription Doses

  • If a service user misses three or more consecutive days of their medication then the client has
  • fficially ‘fell off’ their prescription and should not be dispensed to, as there is a risk of overdose

due to a reduction in tolerance levels. If this occurs and the service user presents then please advise them to contact their recovery coordinator or present to the duty team.

identifiable information (i.e. will only show first part of postcode)
