Network 3500 Covered Services
Network 3500 Plan* The Spirit Network plan pays for covered dental expenses for network, and non-network providers*, based on the contracted fee amount negotiated after the $100 lifetime deductible has been satisfied

- 100% for Preventive services, 50% for Basic, 25%
for Major in year one, with a $1,200 annual maximum, and 10% coverage for Ortho Services
- In year two, Basic Services increase to 65%, 50%
