SBIRT - Concept and Design

¨ SBIRT uses a public health approach to universal screening

for substance use problems.

¨ SBIRT provides: ¤ Immediate rule out of non problem users; ¤ Identification of levels of risk; ¤ Identification of patients who would benefit from brief

advise;

¤ Identification of patients who would benefit from higher

level of care, and;

¤ Progressive levels of clinical interventions based on need

and motivation for change.

Primary Goal

¨ The primary goal of SBIRT is not to identify those

who have a substance use disorder and need higher levels of care.

¨ The primary goal of SBIRT is to identify those who

are at moderate or high risk for psycho social or health care problems related to their substance use choices.

Screening Brief intervention and Referral to Treatment (SBIRT-II)

¨ Is a comprehensive, integrated, public health

approach to the delivery of early intervention and treatment services with substance use disorders;

¨ Is accessed in primary care centers, emergency

rooms, trauma centers and criminal justice settings to provide opportunities for early intervention with at risk substance users before more severe consequences occur.

Highlights of Illinois Model

¨ Federally funded 5 year grant ¨ Based in Illinois- Cook County ¨ Collaboration with Illinois DASA, TASC, Aunt

Martha’s and ACCESS, Light House, State’s Attorney’s Office and Substance Abuse treatment providers.

Point of Interest

¨ No more than 15% of the expanded SBIRT services

are to be provided to adults involved in the criminal justice system within non correctional community settings located in the same geographic area.

Need for SBIRT Services Among CJ Involved Persons

¨ The criminal justice system is a major catchment

area for people with high risk alcohol and drug use behaviors.

¨ DOJ data demonstrates more than 80% or

arrestees have used alcohol or illegal drugs within 3 days of arrest.

¨ More than 60 % of jail and prison inmates have

been found to meet dependence criteria.

Why SBIRT-II in Criminal Justice Settings?

¨ Prevalence of alcohol and other drug problems/

use in criminal justice

¨ Opportunity to identify risk and intervene early ¨ Relevant to rehabilitate & recidivism ¨ Screening as brief intervention

Illinois TASC Model

¨ Targeted area in the CJ system where existing

programs for service/linkages do not exist.

¨ Wanted to screen defendants who were getting no

screening and/or assessment.

¨ Collaborated with the State’s Attorney office to set up

model

¨ Collaborated with Pre-trial Services to set up process

Tools Settings

SBIRT in Illinois operates in the following criminal justice settings:

¨ Courthouses (Urban and suburban settings) ¨ Felony and Misdeamenor Courtrooms ¨ A.M. and P.M. court calls ¨ 5 Courtrooms with rotating Judges

Staff operate within the courthouses in a private area.

Population

¨ Adult offenders with no previous felony convictions ¨ Charged with a specific non violent offense ¨ Majority male clients ¨ Age range 18-70 years old ¨ Mean age 23 years old ¨ Language - primary English

¨ MDPP - Misdemeanor Deferred Prosecution

¨ DPP - Deferred Prosecution Program

Goals of SBIRT in CJ System

¨ To provide effective early intervention to defendants ¨ To link people to treatment services based on individual

need(s).

¨ To educate the criminal justice system on the prevalence

  • f drug use/abuse/addiction within the courts.

¨ To assist Illinois in preparation for health care reform.

Advantages

¨ Screening quickly assesses the severity of substance use

and identifies the appropriate level of treatment

¨ Brief intervention focuses on increasing insight and

awareness regarding substance use and motivation toward behavior change

¨ Referrals to treatment provides those identified as

needing more treatment with access to specialty care.
