What is SASS? Screening, Assessment and Support Services

ØStatewide Program that provides services for children and adolescents who are in crisis and may be in need of intensive services or inpatient hospitalization ØCooperative Partnership that created a single statewide system whose care requires public funding from one of these three agencies:

üDepartment of Children and Family Services (DCFS) üHealthcare and Family Services (HFS) üDepartment of Human Services (DHS)

Who can receive SASS Services?

ØAny youth that is presenting in a psychiatric crisis

AND

ØAge and Financial eligibility

üChildren under the age of 18 seeking public funding for psychiatric services through DHS üChildren and adolescents under the age of 21 enrolled in HFS’ medical programs üAny person for whom DCFS has legal responsibility

What is considered a crisis? ØRisk of harm to self

ØYouth indicating they have been having thoughts of suicide ØPassive: “It would be better if I weren’t here”, “No one would miss me if I was gone” ØDirect: “I want to die”, “I’ve been thinking about killing myself” ØDiscussion of plan and means to carry out the plan ØHave they thought of how they would do it, when they would do it, and have they taken steps to follow through ØSelf harm behaviors ØCutting, burning, or other ways to “harm”, not complete suicide

What is considered a crisis? ØRisk of harm to others

ØYouth indicating they have thoughts to hurt others ØPassive: “They’ll be sorry”, “He/she will pay for that” ØDirect: “I’m going to beat him up”, “I’m going to kill him” ØDiscussion of plan and means to carry out the plan ØSpecific time/way/manner they will hurt someone ØAggressive behaviors ØIntentional vs. unintentional aggression ØDiscussion of past history of aggression

What is considered a crisis? ØOther behavioral health crisis

ØPsychosis ØDecompensation in functioning ØHigh risk behaviors that may put self or others at risk of harm

How are services accessed when imminent risk is suspected? CRISIS AND REFERRAL ENTRY SERVICES (CARES) 800-345-9049

ØSingle point of entry hotline for all SASS calls throughout the state ØCARES staff complete the following:

ØA brief acuity assessment to determine need ØAssessment for age and financial assessment criteria ØReferral to the SASS provider where the youth is presenting in crisis

As the referral source, be prepared to provide:

ØInformation that may be asked by CARES:

ØDemographic information (Name, DOB, Address, Phone, etc) ØLocation of the crisis ØImmediate threat to safety ØPresenting problem ØDuration of symptoms ØParent/guardian information and availability

ØPlease make sure that parents are on site if at all possible to ensure the SASS screener can complete disposition (safety plan, hospitalization). If not available to be on site, please inform them of the call to CARES and ask they be available by phone to consult with screener

When to Call 911 instead of CARES Immediate safety risk:

ØYouth is in crisis that requires immediate response

ØUnable to ensure safety of youth in crisis for the 90 minute window for screener to respond on site to where the crisis is occurring ØUnable to monitor (have eyes on the youth) until screener arrives ØYouth has made an attempt or is in the process of harmful behavior ØEnvironment does not allow for a safe screening ØCaller feels that the situation is unsafe and requires police involvement

What services are offered? SCREENING

Ø90 minute response from the time of call from CARES to have a screener on site where the youth is presenting in crisis ØSASS Screening and Risk Assessment to determine if there is a need for inpatient hospitalization or whether they can contract for safety ØScreening completed by an Emergency Services Counselor ØFull Risk Assessment to determine appropriate least restrictive level of care ØCollaboration with referral source, family, youth and any other professionals involved in care to determine best plan of action

What services are offered? SCREENING

ØHospitalization

üSASS screener will work with parent/guardian to determine the most appropriate hospital in the area üFacilitate admission üCoordinate transportation

ØCommunity Stabilization

üDevelop a comprehensive safety plan to aid in stabilization of symptoms in the home üProvide follow up within 48 hours of initial evaluation

What services are offered?

SUPPORT SERVICES

ØAll clients referred through the CARES hotline are eligible for up to 90 days of community based services that include: ü24-Hour crisis intervention ü Assessment and Treatment Planning üCollaboration and discharge planning with inpatient facility (if applicable) üCare coordination with any existing mental health providers üCare coordination with other family supports üIntensive individual and family therapy/counseling üPsychiatric evaluations and medication monitoring üCase management üLinkage to long term services as appropriate üParent support group

Family Resource Developer

The SASS program has a Family Resource Developer on staff to provide support to families and assist them in achieving the best possible outcomes for their child. ØExperience as a parent, caregiver, or guardian who has gone through a crisis with their own child ØAssists families in identifying resources in their community based on needs of the child and family ØAnswer questions and provide support
